A method of performing a transperineal biopsy procedure with a transperineal biopsy guide secured to a transrectal probe configured to provide imaging. The method includes adjusting an angular orientation of a platform of the transperineal biopsy guide relative to the transrectal probe to which the transperineal biopsy guide is attached. The platform is pivotally coupled to a stabilizing structure of the transperineal biopsy guide. The stabilizing structure is operably coupled to a mount attachable to the transrectal probe. The method further includes inserting a tubular structure into the perineum of the patient, so the tubular structure penetrates the perineum, the tubular structure being supported by the platform and positioned within the sagittal imaging plane. The method further includes inserting a biopsy instrument into the prostate of the patient through the tubular structure penetrating the perineum of the patient.
