Each intake extension tube communicates with an intake lumen of the catheter to provide fluid communication with the respective intake lumens. The intake extension tubes are positioned in a stacked arrangement, and the clamping member has a plurality of posts to receive the stacked tubes to limit lateral movement thereof.
