The at least one barcode is encoded to contain at least a portion of personal data associated with an owner of the personal information object. The portion of personal data comprises at least one of the following declarations: a waiver of privacy rights, an advanced directive declaration, a Do Not Resuscitate (DNR) declaration, an Allow a Natural Death (AND) declaration; and a Do Not Incubate (DNI) declaration. The personal information object further comprises a signature for authorization of at least one of the declarations.
