The method includes receiving, via the communications network, an exercise regimen for a patient, wherein the exercise regimen includes a normalized model of at least one exercise activity, transmitting, via the communications network, the exercise regimen to a computing device of the patient, receiving, via the communications network, patient observation data, including a log of the patient performing the least one exercise activity, and generating a report including data from the exercise regimen and the patient observation data.
