A form (10), such as a mailing form (10), constructed on a single laminated sheet of standard paper size, which is printable by a single pass through a standard non-impact printer, and extendible to provide additional surface area for the form (10) exceeding standard paper size. The extendible form (10) can be adapted for use with mailing forms such as multi-part collated unit sets. The form is useful for providing a Customs Declaration and Dispatch Note having each part printable by a single pass through a non-impact printer.
