    摘要: A cardiac pacemaker (10) has a sense amplifier (20) responsive to signals representative of cardiac activity for passing signal components lying in a selectively variable pass band and for selectively varying the gain of the passed signal. A pair of signal comparators (58, 59) constituting switched capacitor amplifiers, associated with the sense amplifier (20), are responsive to the filtered and gain adjusted signal for comparison of its magnitude to predetermined target levels for determination of heart rate and the need for change of rate. Two target levels are provided in each comparator (58, 59) for comparison with the level of the detected signal, by multiplexing the switched capacitors. Another switched capacitor amplifier (99) is used to develop a voltage reference level insensitive to temperature, supply voltage, and circuit components, for deriving the target levels for the comparison. Still another multiplexed switched capacitor amplifier comparator (125) is used to develop a first target level to regulate the level of the pacing stimulus, and a second target level for comparison with the supply voltage level of the pacemaker as a measure of the point at which the pacemaker requires replacement.

A detected abnormal tachycardia is then treated with any of a multiplicity of therapy regimens of differing degrees of aggressiveness, toward terminating the detected tachycardia.

    摘要: An electrode (10, 35, 50, 65) for use in cardiac pacing has a substrate (15, 87) composed of a material conventionally employed for pacing electrodes, and a surface layer or film (20, 75, 85, 95) of iridium oxide overlying the substrate. For use as a stimulating cathodic electrode and a sensing electrode, the iridium oxide layer is arranged to be in cardiac tissue stimulating relationship when the electrode is in proper position with respect to the patient's heart. The electrode impresses electrical stimuli on the excitable myocardial tissue to provide pacing at very low stimulation thresholds. At the completion of each stimulus, the electrode is capable of abruptly sensing, within an interval less than 100 ms thereafter, the electrical activity of the heart in response to the stimulus to reliably verify capture. The surface of the electrode may be provided with recesses (65) to which the iridium oxide layer may be confined. An iridium oxide layer may be provided on both the cathode and the anode for effecticient transduction at the electrode-electrolyte interface formed by the surface of the iridium oxide layer and the surrounding body fluid and tissue.

    摘要: A medical interventional device (13) is structured for implantation in a human patient (10), to respond to detection of cardiac dysrhythmias. Dysrhythmias in either the atrial or ventricular chambers (24, 29) are automatically treated by a selected therapy regimen consisting of pacing, cardioverting or defibrillating waveforms of predetermined type and energy content to the chamber diagnosed as that in which the dysrhythmia originated. The device incorporates a DDD or DDD-R pacemaker (not numbered) for dual chamber sensing of electrical (ECG) activity, and for constant atrioventricular synchronization. A microprocessor-based logic subsystem (not numbered) diagnoses the origin of a rhythm disorder and identifies the originating heart chamber. The electrode system (21, 26, 42, 38) for delivering the selected therapy includes a single lead (22, 27) for pacing, sensing, cardioversion and defibrillation associated with each of the right atrial and ventricular chambers (24, 29). The device's metal case (15) may be used as the defibrillator's counter-electrode. A cardiac electrode (42) implanted in the distal coronary sinus improves defibrillation and left heart stimulation for better hemodynamic response. Dual chamber rate responsive pacing further improves hemodynamics.
