A method and apparatus are provided for improving the detection of certain cardiac rhythms by combining dual chamber interval-related detection methods with electrogram morphology analysis. A prioritized set of rules are defined wherein each rule is directed at identifying a particular arrhythmia or type of arrhythmia. Each rule includes clauses that may be related to sensed event intervals and interval patterns and at least one rule includes at least one clause relating to the EGM morphology.

In a medical device that monitors and/or treats ventricular arrhythmia, the invention provides for a 'normal' processing mode of operation and a 'guarded' processing mode of operation. The transition between the two modes is a function of cardiac rhythm. Benign rhythms are processed in the normal processing mode and non-benign rhythms are processed in the guarded processing mode. In normal processing mode, the device performs functions that are not computationally demanding, but in guarded processing mode, the device performs the computationally demanding operations that classify arrhythmias. If therapy is deemed necessary, the device may provide the therapy in guarded processing mode.

An implantable anti-tachyarrhythmia device which delivers anti-tachyarrhythmia therapies to a patient's heart in response to detection of tachyarrhythmias. The device defines first criteria indicating the presence of atrial tachycardia and second criteria indicating the presence of artrial fibrillation, and compares the time elapsed since the either the first or second criteria where initially met to a defined time duration. In response to the defined duration having passed and either of the first or second criteria being met, the device triggers delivery of an appropriate anti-tachyarrhythmia therapy. The timer is reset on detection of termination of artrial tachyarrhythmia, but not on failure of the first and second criteria to be met. The first and second criteria are defined such that they can not be concurrently met.

In some examples, a medical device determines that a cardiac rhythm of the patient is treatable with a therapy, such as a defibrillation pulse, based on a cardiac electrogram (EGM). The medical device analyzes detected heart sounds, and withholds or allows the therapy based on the analysis of the heart sounds.

In one embodiment, an initial set of single chamber pacing pulses are delivered in a single one of the atrium and the ventricle, the other one of the atrium and the ventricle being a non-paced chamber during the initial set of single chamber pacing pulses. The device detects simultaneity between a sensed event in the non-paced chamber and one of the single chamber pacing pulses and delivers ATP in both the atrium and the ventricle in response to detecting the simultaneity.

A system and method are provided for assessing T-wave alternans (TWA) using cardiac EGM signals received from implanted electrodes. A T-wave signal parameter is measured from signals received by an automatic gain control sense amplifier. A TWA measurement is computed from a beat-by-beat comparison of T-wave parameter measurements or using frequency spectrum techniques. The TWA measurement magnitude and measurement conditions are used in detecting a clinically important TWA. TWA assessment further includes discriminating concordant and discordant TWA in a multi-vector TWA assessment, and determining the association of a TWA measurement with QRS alternans, mechanical alternans, and other physiological events. A prediction of a pathological cardiac event is made in response to a TWA assessment. A response to a cardiac event prediction is provided.
