    摘要: A duckbill check valve and septum combination includes a hollow elastomeric body having an open proximal end, a closed distal end and a pair of opposed flat portions that form a distal edge. The distal edge includes a slit having a periphery that is smaller than the periphery of the instrument inserted therethrough. The slit provides a zero seal in the absence of an instrument and an instrument seal in the presence of an instrument. A shield including a tubular member and a plurality of blades protruding distally from the distal end of the tubular member may be placed inside the check valve and operates to center and guide the surgical instrument through the blades before expanding the slit of the check valve. An instrument inserted into the check valve stretches the distal portion of the duckbill valve to form a septum-like seal around the inserted instrument.

    摘要: An insufflating surgical instrument adapted for movement across an abdominal wall to insufflate an abdominal region of a patient is disclosed comprising a shaft having an insufflation channel extending along an axis between a proximal end and a distal end, the insufflation channel being adapted for connection to a source of fluid under pressure at the proximal end, a tip at the distal end of the shaft, and at least one vent hole formed at the tip or the shaft being in connection with the insufflation channel and being adapted to expel the fluid under pressure to insufflate the abdominal region. At least one of the tip and the shaft is formed of a translucent or transparent material such as polycarbonate to facilitate visualization of the abdominal wall and the abdominal region. The shaft includes a lumen extending along the axis between the proximal end and the distal end to enable insertion of a laparoscope. The lumen and insufflation channel may be formed as separate channels or as one shared channel. The tip may be blunt, the shaft and the tip may be integrally formed, and the vent hole may be of any geometric shape including round, oval, square and rectangular. The blunt tip may further comprise a marker to indicate penetration of the abdominal wall. In another aspect, the tip may be sharp, pointed or bladed to facilitate penetration of body tissue.

    摘要: The invention is directed to a trocar adapted to form a seal around a surgical instrument, the trocar comprising a cannula having an axis extending between a proximal end and a distal end; a housing having a proximal wall and forming with the cannula a working channel sized and configured to receive the instrument; a septum seal disposed in the housing and extending transverse to the axis of the cannula across the working channel, the septum seal having a normal state when the instrument is absent from the working channel and a stretched state when the instrument is being withdrawn from the working channel; portions of the proximal wall of the housing defining an orifice sized and configured to receive the instrument into the working channel; and an elastomeric ring-shaped projection extending axially distally between the proximal wall and the septum seal, the ring-shaped projection having an axial length adapted to deform to prevent inversion and binding of the septum seal and the orifice upon withdrawal of the surgical instrument. In particular, the septum seal is prevented from reaching the orifice in the proximal wall of the housing when the instrument is withdrawn. The projection may be bonded, insert-molded, or compressively fitted to the proximal wall of the housing. The projection may be formed of an elastomeric material such as natural or synthetic rubber. The projection may further comprise a plurality of portions or fingers extending axially distally from the proximal wall, each of the extending portions or fingers may further include a living hinge. In another aspect, the ring-shaped projection may be formed on the surface of the septum seal instead of the proximal wall of the housing. That is, the ring-shaped projection may be formed to extend axially proximally from the surface of the septum seal toward the proximal wall of the housing.

    摘要: A stabilization seal is used with existing cannulas for forming a gas-tight seal with tissue in a body wall The stabilization seal includes a cylindrical inflatable elongate tube having a graduated wall-thickness that is thicker in a central region and thinner in a distal-end region. The tube includes a sealing cuff, having an inflation port, for sealing around a cannula. Application of inflation pressure greatly expands the distal-end region while the central region expands slightly. Another embodiment of a stabilization seal includes an inflatable thread that is used with a cannula having a helical channel on its outer surface. An inflatable tube is wound into the channel. A distal end of the tube includes a gas-tight seal and a proximal end of the tube includes an inflation port In the uninflated condition the nested tube is flush with the channel. Inflating the tube enlarges the cannula assembly.

    摘要: The invention is directed to a trocar assembly having a channel defined along an elongate axis, the trocar assembly being adapted to receive a surgical instrument, the trocar assembly comprising a septum seal disposed in the channel including a seal tip having a proximal facing surface, the seal tip including portions defining an orifice; and a septum shield including a tubular member having a proximal end and a distal end, and a plurality of blades or leaflets protruding distally from the distal end of the tubular member, the septum shield being placed inside the septum seal such that the blades engage the proximal facing surface of the seal tip. The trocar assembly may further comprise a zero closure seal such as a double duckbill valve disposed in the channel outside of the septum seal. The septum shield operates to reduce the drag force and to minimize axial movement of the septum shield and the instrument during insertion and removal of the instrument through the septum seal. The septum shield may be formed from a rigid plastic material, and the septum seal may be formed of an elastomeric material. The blades or leaflets may overlap or offset one another. The orifice may be expandable to accommodate the instrument having a diameter of about 5-15 mm. The blades or leaflets have distal tips that glide or roll against the instrument during placement of the instrument. The distal tips may comprise of a combination of material, durometer and shield geometry to control the behavior of the septum shield. The trocar assembly may further comprise a second septum shield disposed outside of the septum seal.

    摘要: The invention is directed to a surgical wound retractor for retracting and sealing an incision and forming a functional opening or channel through which a surgical procedure may be executed. The wound retractor provides a path for a surgeon to insert his hand and/or instruments through the opening formed by the wound retractor. The wound retractor is sized and configured to be easily placed through a small incision and removed without further insult to the body tissue adjacent to the incision. The wound retractor is adapted to dilate a surgical wound incision to a desired diameter, and comprises a first ring having a diameter greater than the desired diameter of the wound incision and being adapted for disposition interiorly of the wound incision; a second ring having an annular axis and a diameter greater than the desired diameter of the wound incision and being adapted for disposition exteriorly of the wound incision; and a flexible sleeve disposed in a generally cylindrical form between the first ring and the second ring, the first ring having at least one notch to facilitate folding or collapsing of the first ring during insertion and removal of the first ring through the wound incision. The first ring may further comprise a second notch disposed on an opposing end of the first notch to further facilitate folding or collapsing of the first ring. With this aspect, the first ring is folded by squeezing between the first and second notches during insertion and removal of the retractor from the incision. In another aspect, the wound retractor may further comprise a tether having a length, a first end attached to the first ring interiorly of the sheath, and a second end disposed outside of the wound incision, wherein the tether facilitates removal of the first ring by pulling on the second end to retrieve the first ring through the wound incision.
