A catheter is disclosed that has a detachable tip for delivery of a therapeutic substance to a chronic total occlusion (CTO) within a body vessel. The tip is made from a biodegradable material and includes an active agent dispersed therethrough. The detachable tip pierces into the calcified lesion, detaches from the catheter, and remains embedded within the lesion as the tip degrades, thereby releasing the active agent. The active agent is a therapeutic substance effective to treat of a CTO. The active agent may be a drug, an enzyme, bacteria, or a parasite that degrades or softens the calcified lesion.

A cryotherapeutic system configured in accordance with a particular embodiment can include an elongated shaft having a distal portion and a pressure-relief portion proximal to the distal portion. The cryotherapeutic system can further include a supply lumen, an exhaust passage, and a balloon configured to receive refrigerant from the supply lumen and to exhaust refrigerant to the exhaust passage. The pressure-relief portion can be configured to release refrigerant from the exhaust passage when a pressure of refrigerant in the exhaust passage exceeds a threshold pressure less than a pressure rating of the balloon. The pressure-relief portion, for example, can include a rupture element configured to rupture at about the threshold pressure.

A movable-core guidewire including a proximal portion and a distal portion. The proximal portion includes a proximal hollow tube. The distal portion includes a coil coupled to the proximal hollow tube and an elastomeric distal tube disposed within the coil. A core wire is disposed within the hypotube. The core wire is movable between a distal position wherein a portion of the core wire is disposed in the distal tube and a proximal portion wherein the core wire is removed from the distal tube. A method is also disclosed for advancing the guidewire to the treatment site.
