摘要: A cardiac ablation instrument capable of removing blood from a treatment area is provided. The instrument includes a catheter configured to deliver a distal end thereof to a patient's heart. The instrument can also include an expandable element coupled to the distal end of the catheter wherein the expandable member is configured to be positioned adjacent a target area thereby defining a treatment area between the expandable member and the target area. Further, the instrument can include an irrigation mechanism configured to dispense an irrigation fluid from the catheter thereby displacing blood from the treatment area. Additionally, the instrument includes an energy emitter configured to deliver energy to tissue within the treatment area. The instrument can also include a contact sensor configured to determine the presence of such blood within the treatment area. Methods for ablating tissue are also provided.

摘要: A cardiac tissue ablation system includes an elongate catheter configured to deliver a distal end thereof to a patient's heart and an expandable member formed at the distal portion of the catheter. The expandable member has an elastic portion configured to conform to the shape of a target tissue region upon expansion. The expandable member has a plurality of inflation settings that correspond to different levels of inflation of the expandable member. The system also includes a controller for selecting an inflation setting for the expandable member and an energy emitter movably disposed within a lumen of the catheter. The energy emitter is configured to deliver radiant ablative energy to the target tissue region.

摘要: Methods and apparatus are disclosed for forming annular lesions in tissue. The methods include introduction of an optical apparatus proximate to a tissue site, via, for example, a catheter. The optical apparatus includes a pattern-forming optical wave guide in communication with a light transmitting optical fiber. Energy is transmitted through the optical fiber, such that radiation is propagated through the optical fiber and the wave guide projects an annular light pattern, e.g., a circle or a halo.

摘要: The present invention provides devices and methods for the treatment of atrial fibrillation. In one embodiment method for treatment of atrial fibrillation is provided that includes positioning a sheath catheter having a deflectable distal end segment in the left atrium of a heart, orienting said sheath catheter such that an ablation instrument can be delivered through the sheath catheter to a position proximal to a first pulmonary vein, activating the ablation instrument to form a circumferential lesion around the first pulmonary vein, repositioning the sheath catheter by deflecting the distal end segment to another orientation, and activating the ablation instrument to form a circumferential lesion around at least one additional pulmonary vein.

摘要: Laser energy produced by a laser operating in the mid-infrared region (approximately 2 micrometers) is delivered by an optical fiber in a catheter to a surgical site for biological tissue removal and repair. Disclosed laser sources which have an output wavelength in this region include: Holmium-doped Yttrium Aluminum Garnet (Ho:YAG), Holmium-doped Yttrium Lithium Fluoride (Ho:YLF), Erbium-doped YAG, Erbium-doped YLF and Thulium-doped YAG. For tissue removal, the lasers are operated with relatively long pulses at energy levels of approximately 1 joule per pulse. For tissue repair, the lasers are operated in a continuous wave mode at low power. Laser output energy is applied to a silica-based optical fiber which has been specially purified to reduce the hydroxyl-ion concentration to a low level. The catheter may be comprised of a single optical fiber or a plurality of optical fibers arranged to give overlapping output patterns for large area coverage.

摘要: A method for maintaining a balloon of a balloon catheter at a prescribed pressure of between about 0.2 psi and 1 psi comprises the step of: generating a vacuum within a reservoir defined in a burette that is part of a fluid management system that is configured to controllably inflating and deflating the balloon by circulating balloon fill media along a fluid circuit, wherein generating the vacuum results in formation of a pressure differential along the fluid circuit, thereby allowing the balloon to be maintained at the prescribed pressure between about 0.2 psi and about 1 psi.
