A sling for supporting a urethra in a patient has characteristics for resisting pull-out, roll-over and a combination of both pull-out and roll-over. To facilitate resistance to pull-out, sling material has protuberances or serrations to resist movement once placed in the patient by the surgeon. Improved resistance to roll-over or roll-up is achieved by providing relief on the sling in areas potentially subjected to roll-over forces by endopelvic fascia. The sling may be used on female or male patients, and may be used in conjunction with a wider support portion to support a bladder and a urethra, rather than a urethra alone. The sling may also be used to support other tissues within a patient.

A medical device 10 for collecting both endocervical cells and exocervical cells from a human or veterinary patient includes an elongate handle 12 having first and second handle ends 14 and 16, an elongate foam portion 18 carried by one of the handle ends 14 or 16, and a disk-like foam portion 19 carried by either the same or the opposite handle end 14 or 16. The device 10 includes at least one base plate 30 or 32 connected to the handle 12, to which the elongate and disk-like foam portions 18 and 19 are affixed. The elongate foam portion 18 can be cylindrical in shape, while the disk-like portion 19 can be a convex disk 20 or a generally flat disk 60 or 80. The first foam portion 18 is positioned about a tip 40 or 48 carried by the first handle end 14, for example, connected to the first base plate 30. The first and second foam portions 18 and 19 can be carried on opposite ends 14 and 16 of the handle 12, or can be unitarily formed as a single foam portion 22 carried by the first handle end 14. The foam portions 18, 19, 20, 22, 60 and/or 80 comprise an open cell, reticulated foam (such as a polyester polyurethane foam of relatively large mean pore diameter, for example, 1 mm or larger) which is sufficiently pliant to take the shape of the internal and external surfaces of the cervix and is sufficiently abrasive to scrape an acceptably large number of cells from the internal and external surfaces of the cervix during use. In an alternative embodiment, the handle 12 includes a lumen 65 defined therein which is dimensioned to receive an endocervical brush 84 or other structure or object 82 therein, such as a guide wire, an endoscope, an optical fiber or a sensor. The device 10 can include the brush 84 or other object 82. The foam portion in that embodiment is preferably a perforate disk 80. The device 10 enjoys a superior combination of cell capture and relative patient comfort.

An embryo transfer catheter with a protective sheath is disclosed. The use of a protective sheath shields a guide catheter from cervical mucus. The retracted open flaps of the outer protective sheath draw the mucus and blood away from the inner guide catheter. This allows an inner embryo transfer sheath to enter the uterus unobstructed. The embryo is then transferred to the uterus by means of fluid pressure with a culture medium. The protective sheath may be snap fit onto the guide catheter. The sheaths and catheters may be manufactured in a variety of lengths for individual convenience and then snap-fit onto each other for better control during an embryo implantation procedure. Other catheters and related instruments may also be manufactured with snap-on features.

A method of performing extracorporeal shock wave lithotripsy using a device which is a quadruple lumen catheter with one through lumen exiting at an end hole to allow the catheter to slide over a wire guide, a second lumen serving to inflate and deflate a balloon located near the distal tip of the catheter, a third lumen used to inflate and deflate a proximate balloon, a fourth lumen to supply a liquid to be delivered between the balloons and radiopaque markings for determining the position of the balloons of the catheter through X-rays of fluoroscopy. The catheter is inserted over a wire guide so that the distal balloon is located on one side of the stone in the ureter and the proximate balloon is located on the other side whereupon the balloons are inflated and the cavity formed by the balloons and the wall of the ureter is filled with fluid to surround the stone with fluid which aids in the pulverization of the stone by extracorporeal shock waves. The pulverized fragments of the stone are removed by deflating the proximate lumen and forcing water or another fluid through the fluid supply lumen. The fragments are prevented from migrating into the kidney by the distal balloon which remains inflated. The proximate balloon may be reinflated and the cavity filled with fluids and subjected to extracorporeal shock waves again to further pulverize the stone and may be deflated so that the stone fragments may be flushed down the ureter as necessary.
