    摘要: An instrument for delivering a variable pressure stream of fluid as a coherent jet. The instrument may be configured for holding in the hand of a user or for manipulation by machine control. The orifices may be oriented axially, transversely or obliquely. A deflector may be provided opposite the orifice to transform the jet stream into a harmless spray. The deflector may be slidable (in the axial mode) or pivotable (in the transverse mode) to ensure a small instrument profile. The instrument may be moldable allowing the user to reshape the jet tip into a desired configuration. The instrument may be remotely steerable and enable a user to selectively alter the direction of the jet without having to remove the instrument from the surgical site.

    摘要: A mechanism advances to stretch an intraaortic balloon prior to insertion or removal. One embodiment includes a member which extends through a catheter from the handle to the balloon, and which advances to extend the balloon without appreciably twisting it. A spring detent assembly in the handle defines a precise amount of extension delivered by the member. The mechanism permits the balloon to be fully relaxed once it is inserted and to be recompacted, following use, for removal from the patient. The device is of particular utility for a large diameter, irregular or polygonal non-cylindrical balloon, and for inserting a balloon along an arterial path having a narrow passage or sharp curve.

In some embodiments, the invention provides a support device which includes a bladder capable of containing a fluid and a post adjacent the bladder. In some embodiments, the invention provides a post which includes a reduced cross-sectional area in comparison to an adjacent region of the post such that the resistance of the rolling diaphragm portion of the bladder to rolling movement along the post due to the applies force decreases as the bladder rolls along the reduced cross-sectional area region.     摘要: The invention provides a variety of surgical instruments for forming a liquid jet, which are useful for performing a wide variety of surgical procedures. In some embodiments, the invention provides surgical liquid jet instruments having a pressure lumen and an evacuation lumen, where the pressure lumen includes at least one nozzle for forming a liquid jet and where the evacuation lumen includes a jet-receiving opening for receiving the liquid jet when the instrument is in operation. In some embodiments, the pressure lumen and the evacuation lumen of the surgical liquid jet instruments are constructed and positionable relative to each other so that the liquid comprising the liquid jet, and any tissue or material entrained by the liquid jet can be evacuated through the evacuation lumen without the need for an external source of suction.     摘要: The present invention provides a series of devices useful for surgical procedures utilizing rotatable components for grinding, cutting, ablating, polishing, drilling, screwing, etc., tissues of the body of a patient. The invention includes, in one aspect, a series of devices comprising surgical instruments including rotatable shafts, and surgical components drivable by the shafts that can be utilized for contact with tissue in a surgical operating field. Some preferred surgical instruments provided by the invention utilize a liquid jet-driven rotor mechanism for driving rotation of the rotatable shaft. Some preferred instruments provided by the invention include both a liquid jet-driven rotor mechanism and a nozzle at the distal end of the instrument for forming a liquid cutting jet for cutting or ablating tissue of a patient. Such instruments can include a liquid flow directing valve therein that includes a pressure-tight sealing component comprising a sealing element that is constructed and arranged to be slidably moveable within a cylinder of the valve. The invention provides methods for utilizing the inventive surgical instruments in surgical procedures involving both cutting or ablating tissue of a patient with a liquid cutting jet and grinding, cutting, or ablating tissue with a rotating surface of a surgical instrument.

    摘要: A periodontal probe has a probe finger extending from a handle to an L-shaped distal end which includes a tip adapted for insertion into a periodontal pocket and formed of a thermally insulative material. A temperature sensing element is mounted at the extreme end of the tip. Conductive leads extend from the sensing element, through the probe tip to the handle. In a preferred embodiment, the temperature sensor is a thermocouple or thermistor, and the tip is sealed within an epoxy outer skin. The tip tapers so that the thermal mass of the probe in the vicinity of the sensing element quickly attains the temperature of surrounding tissue while minimally perturbing that temperature. The handle may contain signal processing or temperature display units, and the probe tip preferably includes graduations for indicating probe depth. Interchangeable sensor matching circuit elements adapt sensor elements having different characteristics to a common display driver. A system includes a probe and a processor with a display which indicates periodontal site locations to prompt entry of temperature measurements and to display disease indications based on those measurements. The temperatures are processed and are compared to stored tables of temperature difference distributions, to provide site-specific tissue diagnosis.
