    摘要: A method and system for identifying systemic failures and a root cause of incidents in a work environment. Failures, causes, and impacts related to a sample of incidents can be investigated in order to identify one or more systemic failures and root causes of the systemic failure(s). The incident details can be utilized to identify one or more root cause manifestation for each root cause of the systemic failure(s). The gaps in an existing system can be analyzed and one or more improvement solutions based on corrective actions can be identified. The improvement solutions with a highest priority can be selected and implemented to address the gaps identified in the current system based on the analysis of systemic failures and root causes. The effectiveness of the improvement solutions can be tracked and monitored to prevent the recurrence of future incidents.

    摘要: An equipment area alarm summary display system and method that can help operators cope with inevitable alarm floods is disclosed. An equipment area summary display with dedicated screen areas for each major equipment area works together with a detailed list display. Alarms within each equipment area can be grouped logically and can be shown via an icon with the most recent alarm indicators showing at the top of the display for each area. A specific alarm indication in the equipment area summary display can be selected so that a corresponding item can be highlighted in the detailed list display, if the alarms are active. Thus enabling the operator to minimize the time needed to develop awareness of the flood situation and intervene to get the process back under control.

A set of rows in the alarm trend list can be organized in a top-to-bottom manner. The set of alarm indicators can be aligned with most recent alarms appearing on a right side of the alarm trend list. The specific alarm indicators can be selected in the alarm trend list, so that a corresponding item can be highlighted in the alarm summary list, if the alarms are active. Hence, the system and method can allow an operator to see when alarms occur, their sequence, relations and priority, which aid the operators cope with inevitable alarm floods.

    摘要: A method of continuously improving a procedure management system that generates human operating procedures. At least one failure mode (“root cause”) is defined for each human procedural execution failure. The root causes are mapped to stages of human operator execution for the human operating procedures where the plurality of root causes manifest, and to human intervention activities that the human operator failed to correctly execute (“procedure deficiency”). Each root cause is mapped to one or more procedure management components that may have generated the procedure deficiency. Recorded actual incidents of human procedural execution failures are analyzed to identify at least one assignable root cause. A procedure management component is updated to mitigate the assignable root cause, and is then used to change at least one human operating procedure or to generate a new human operating procedure to reduce an incidence of the human procedural execution failures.
