A method and apparatus for detecting and optionally treating metabolically unstable or active atherosclerotic plaque is provided. The patient is given an I.V. solution containing a radioactive tracer which specifically accumulates in these plaques. A miniaturized radiation detector for local radioactivity imaging and detection is applied directly within the arterial lumen. The radiation detector identifies and differentiates the active, unstable plaque from the inactive, stable plaque. Treatment may optionally be provided through any of three forms, including opening the vessel by mechanical expansion, removing the plaque by mechanical ablation, or delivering a metabolism-altering agent to the affected site.

The invention further relates to a specially designed intravascular device that delivers excitation light to the plaque surface inside the artery and receives emitted fluorescence light when a fluorescent signal is detected.

A probe or sheath for use on a probe for detecting and removing radioactively tagged tissue, e.g., a sentinel lymph node, within the body of a living being. The probe or sheath and probe is arranged to be inserted through a small percutaneous portal into the patient's body and is movable to various positions adjacent the tagged tissue to detect the presence of radiation therefrom so that it can be positioned immediately adjacent that tissue to ensnare or trap that tissue. The probe or sheath and probe can then be removed from the being's body, carrying the tagged tissue with it. The probe may be constructed to make use of a scintillation crystal, a collimator, adjustable or fixed, and a backshielding lightpipe. A blunt dissecting device is provided as part of the sheath or as part of the probe itself to aid in separating the tagged tissue from adjacent tissue without injury to the adjacent tissue. The probe may be a fully integrated, self-powered unit.

A crystal containing detector particularly suited for use in immunoguided surgery capable of detecting gamma emissions present at tumor sites. The instrument utilizes a cadmium-zinc-telluride detector crystal which may be retained within a finger mount positioned over the surgeon's finger. Within this mount there also is incorporated a first preamplification stage designed to accumulate charge signals from the detector as well as to drive a relatively short length of cable to a second preamplification stage located, for example, within a housing at the surgeon's upper arm. The small finger mounted probe may be moved between operative orientations at the underside of the tip of the finger to stand-by orientations at the top of the finger adjacent the knuckles. In an additional embodiment, the finger probe incorporates a sodium iodide crystal, the scintillations from which are transmitted via a short length of fiber optic cable to an upper arm mounted housing which contains a photomultiplier tube. Signals from the remote, arm-mounted housing then are transmitted via longer cabling or the like to a control console.
