A method and apparatus for enabling access to an intramedullary canal of a femur through a femoral knee joint prosthesis includes a first condylar portion and a second condylar portion. The first condylar portion has a first femoral bearing surface and the second condylar portion has a second femoral bearing surface. An inner condylar portion extends between the first condylar portion and the second condylar portion and defines an opening passing therethrough. A seal member is operable to substantially seal the opening such that the seal member is further operable to be opened to enable access to the intramedullary canal of the femur without having to remove the femoral knee joint prosthesis from the femur. A method for enabling access to the intramedullary canal of the femur through the femoral knee joint prosthesis is also provided.

The insert may comprise a member of the calcium phosphate family. The spinal cage insert may be made to a desired shape of porous ceramic, and it may include channels and/or surface features. Various shapes of filler pieces are also provided, wherein the filler pieces may be suitable to augment external regions of vertebrae which have been fused to each other so as to promote build-up of bone. The spinal cage insert and/or the filler pieces may be osteoconductive and may also contain osteoinductive substances or material. The articles may also contain cavities suitable for containing particles of demineralized bone matrix (DBM). Methods of use and methods of manufacturing the spinal cage insert and filler pieces are also provided.

The humeral portion of a shoulder prosthesis includes a diaphyseal component configured for fixation within the humerus, a metaphyseal component, configured for removable implantation within the metaphysis of the humerus, and an engagement mechanism for removably engaging the two components. The metaphyseal component can initially include a convex articulating surface that can be replaced with a concave surface during the shoulder arthroplasty procedure or in a subsequent revision surgery. The metaphyseal component can also include a feature to facilitate removal of the component from the bone once it has been disengaged from the diaphyseal component.

The outer wall has pores to permit bone tissue to grow into the outer wall, and the inner surface of the shell engages the socket. The inner wall has access channels suitable for seating a bone screw. The access channels are closed off by an associated integral section of the shell. Preferably, each access channel is closed off by a section of the outer wall so that the shell retains maximal outer surface area for bone tissue ingrowth. When a number of bone screws are needed for affixation of the shell to bone, a number of locations for bone screws can be created in the shell by opening the closed access channels.

The distal end has a slotted opening (120) extending through the body (100) and forming a gap extending between the two or more bore holes (110) to form a deflectable bridge (102) spanning the bore holes (110). The bridge (102) has a center opening (50) for receiving a set screw (20) passing through the bridge (102) into a threaded opening (52) in the body. When the set screw (20) is tightened, it deflects the bridge (102) closing the gap as the bridge (102) contacts an exterior surface of each head (210) of the polyaxial fasteners (200). The angulation of each polyaxial screw (200) is adjustable, controlled by the tightening of the set screw. Initial contact of the bridge (102) with the head (210) of the fastener (200) reduces angulation movement of the shank (220) requiring manual manipulation. Increased tightening of the set screw (20) fixes the angulation as the bridge (102) reduces the gap and increases fastener head contact forces.
