Abstract: A surgical instrument comprises an elongate probe, in which the probe comprises an endoscope and a treatment channel. In some embodiments, the endoscope comprises a lens, in which the treatment channel extends through the distal end of the lens, which can facilitate alignment of the probe with a target tissue. In some embodiments, the treatment channel and the endoscope may comprise a co-axial configuration, which can decrease parallax when a user views the distal end of the working channel and target tissue beyond the distal end. Alternatively, the probe may comprise lenses arranged to provide stereoscopic vision of the end of the probe. The treatment channel may comprise an optical fiber to deliver light energy or a working channel. The treatment channel may comprise a housing such as a tube extending from the endoscope, in which the endoscope is configured to image the end of the treatment channel.

Abstract: Disclosed herein are systems and techniques for compensating for insertion of an instrument into a working channel of another instrument in a surgical system. According to one embodiment, a method of compensation includes: detecting insertion of an insertable instrument into a working channel of a flexible instrument; detecting, based on a data signal from at least one sensor, a position change of a distal portion of the flexible instrument from an initial position: generating a control signal based on the detected position change; and adjusting a tensioning of a pull wire based on the control signal to return the distal portion to the initial position.

Abstract: Systems and methods to facilitate visual and tool access within a volume reached by an endoscopic tool inserted into brain tissue. Cameras are placed in various locations distal to and/or proximal to the volume, and in some cases are repositionable. Provision of different viewing angles helps to maintain close visual monitoring of surgical progress. Retractor and scaffolding are used, in some embodiments, to open and/or maintain opening of a working volume, particularly a working volume from which material is being removed, and which may be prone to partial collapse as surrounding tissue pushes inward into the evacuated area. Working channels are reconfigurable by use of inner and outer parts, and working channels may be shaped to allow simultaneous use by a plurality of tools and/or supports.

The surgical instrument assembly also includes an imaging assembly. The first and second end effectors are within the imaging assembly's field of view.

Abstract: A multifiber assembly and methods of using the same in an endoscopic procedure for transmitting illumination light to, and a response signal reflected from, a target is disclosed. An exemplary device comprises a proximal end, and distal end, and a transition section between the proximal and distal ends. The proximal end includes a first connector to be connected to a light source and a second connector configured to be connected to a spectrometer. The distal end includes a shaft including at least two first optical fibers to transmit light and at least one second optical fiber to transmit a spectroscopic signal. The transition section can couple the first connector to the at least two first optical fibers, and to couple the second connector to the at least one second optical fiber.

Abstract: Methods and devices for generating ultrasound images during a medical procedure are disclosed. The device may comprise a shaft that includes a working channel for receiving an instrument, the working channel extending from a proximal end of the shaft to a distal end of the shaft, and an ultrasound sensor disposed radially outward of a distal portion of the working channel such that the ultrasound sensor at least partially surrounds the working channel. The ultrasound sensor may be configured to image a single field of view greater than about 90 degrees about the shaft, e.g., simultaneously, while conducting a medical procedure with an instrument inserted through the working channel.
