Abstract: A polyaxial bone screw system and method for connecting a longitudinal rod to multiple bones or bone portions. The system may include a bone fastener having a shank and a shank head; a polyaxial body to receive the shank head; and a collet for applying force to exterior surface of the shank head when the collet is acted upon by a rod, which is pressed onto the collet via a set screw received in the polyaxial body. The force applied to the shank head by the collet is adequate to temporarily lock the desired orientation of the shank with respect to the polyaxial body.

Abstract: Systems, methods and devices for providing orthopedic implant augments are provided which have fastener locking mechanisms. The augments include a surface for mating with an implant component and a surface for interfacing with a patient's bone. The fastener locking mechanisms are aligned such that the augment may be locked in a variety of orientations while maintaining close contact with both an implant and a patient's bone. The alignment of the locking mechanisms provides variability and adjustability to the augment to address a variety of bone anatomies and requirements.

The methods of using the fusion systems including exposing a patient's vertebrae and obtaining the fusion system, inserting first and second fasteners into first and second vertebra, and coupling the first fastener and second fastener.

The nail may also have one or more apertures for installing one or more crossing screws through the nail. An installation instrument particularly constructed to install the provided nail is provided. A first and second portion of the provided nail installation instrument may rotate relative to one another when a locking mechanism is disengaged, but are locked in position relative to one another when the locking mechanism is engaged, which enables installation of the provided nail.
