摘要: Surgical techniques and instruments are provided for preparing a disc space for implantation of a vertebral fusion device or implant. In accordance with one embodiment of the technique, a surgical instrument assembly (40) is provided having an outer sleeve (50) with distraction fingers (58) at one end to maintain distraction of a disc space. A switching sleeve (100) having a pair of rotatable distractors (108) is coupled within the outer sleeve (50), and the sleeves are placed over a dilator (150) until the distractor heads (110) are placed within the disc space. The dilator (150) is withdrawn and the disc space distracted by rotating the distractors (108) of the switching sleeve (100). The switching sleeve (100) is uncoupled from the outer sleeve (50) and the fingers (58) of the outer sleeve (50) are then inserted into the disc space. The switching sleeve (100) and dilator (150) are then removed and the outer sleeve (50) defines a channel therethrough that allows insertion of implants at bilateral locations within the disc space without movement or manipulation of the outer sleeve (50). Laparoscopic procedures may be performed in the disc space by placing a sealing member (180) over the end of the outer sleeve (50) after it is inserted into the disc space.

摘要: A plating system for stabilization of a bony segment includes a plate engageable to at least first and second bony elements. For spinal stabilization, the plate is attached at least to first and second vertebrae with at least one bone fastener in each vertebra. Retaining members are attachable to the plate to prevent the bone fasteners from backing out of the plate. One retaining member has apertures over the bone plateholes and allows the bone fasteners to pivot in the plate. A second retaining member fixes the bone fasteners erlative to the plate by means of engagement members which engage the tool engagement recesses of the bone fasteners.

The implants are formed to include a concave surface (14) formed from a portion of the medullary canal of a long bone. The concaved surface defines a recess (15) in the implant that serves as a depot for osteogenic material. Specific instruments for inserting the implants prepared according to this invention and for preparing the intervertebral space to receive the implants are also provided.

摘要: Systems and methods for stabilizing the spine are provided. For stabilizing the L5-S1 junction, the system includes a plate (7) that has a generally triangular shape with an upper node (8a) and a pair of lower nodes (9a, 10a). The upper node (8a) has a hole (8) through the plate (7) to receive a screw (11) for passage into the L5 vertebra. The loer nodes (9a, 10a) each include a hole (9, 10) through the plate (7) to receive screws (12, 13) for passage into the S1 vertebra. The system further includes a plate (7) having a retaining element (32) for preventing backout of screws inserted through the plate (9). Instruments and methods for attaching the plate to the spinal column are also provided.

The instruments include a distractor (18) and a cutting instrument (52). In one embodiment the distractor includes a body portion (20) and a pair of flanges (26, 28) extending along opposite sides of the body portion (20) such that a slot (42, 44) is formed between the flanges (26, 28) and the body portion (20). The cutting instrument (52) is positionable over the body portion (20) and into the slots (42, 44) of the distractor (18) so that the flanges (26, 28) are positioned between the cutting instrument (52) and the adjacent tissue.
