摘要:
The present method involves the photodynamic treatment of acne vulgaris. The method involves the topical administration of a photosensitizer composition comprising hydrophobic green porphyrins such as lemuteporfin, polyethylene glycol and skin penetration enhancers such as oleyl alcohol and TRANSCUTOL™ to acne-affected skin and subsequent exposure of that skin to energy of a wavelength of activating the photosensitizer.

The present invention relates to a solid pharmaceutical product provided in the form of a suppository which comprises a photosensitiser which is a 5-ALA ester of general formula I, or a pharmaceutically acceptable salt thereof:
R 2 2 N-CH 2 COCH 2 -CH 2 CO-OR 1 (I)
(wherein R 1 represents a substituted or unsubstituted, straight-chained, branched or cyclic alkyl group; and each R 2 independently represents a hydrogen atom); and at least one pharmaceutically acceptable carrier or excipient which is a hard fat or a mixture of hard fat and additives. The pharmaceutical product may be used in the photodynamic treatment of cancer, and infections associated with cancer, and in the treatment of non-cancerous conditions, in particular in treating cancerous or non-cancerous conditions in the female reproductive system, for example cervical cancer and infections caused by the human papillomavirus.

The present invention provides an acid addition salt of 5-aminolevulinic acid (5-ALA) or of a 5-ALA derivative (e.g. a 5-ALA ester) with an acid which has a pKa of about 5 or less, preferably about 3 or less, with the proviso that the acid is other than hydrochloric acid. Particularly preferred salts are those derived from acids selected from the group comprising sulphonic acid and its derivatives, hydrobromic acid, sulfuric acid, nitric acid and phosphoric acid. The salts in accordance with the invention are particularly suitable for use as photosensitizing agents in diagnosis and photochemotherapy of disorders or abnormalities of external or internal surfaces of the body.

The invention discloses compounds and compositions for dual phototherapy and combined therapy and diagnosis of tumors and other lesions. The compounds have a Dye that, when photoactivated, operates via Type I and/or Type II mechanisms. Other Dye or azide components may operate by the same or different mechanisms. Selection of particular components in a compound, and formulation of the compound(s) in a composition permit different activation wavelengths to be used for different therapies. A targeting moiety may be added to the compound or composition so that the Dye locates at a particular site, such as a hormone-sensitive tumor, for diagnosis and/or treatment. The compounds and compositions may be incorporated within liposomes.

Light treatments of sebaceous gland disorders with 5-aminolevulinic acid and photodynamic therapy are disclosed. A preferred treatment includes topical application of 5-aminolevulinic acid to the skin followed by light exposures with repeated treatment at various intervals. At low doses of ALA and photodynamic therapy (PDT) in single or multiple treatments, improvement in the sebaceous gland disorder, e.g., acne, provides the discovery that diminishment in sebum secretion and the eradication of bacteria occurs. At high doses of ALA and a single high energy PDT treatment, permanent changes to the sebaceous gland and sebum secretion have been discovered.

The invention relates to the use of porphyrin synthesis substances, in particular 5-amonolevulinic acid, if necessary in combination with salicylates in the form of acetylsalicyc acid and with antioxydising agents in the form of ascorbic acid when a phototherapy is carried out with a light emission whose wavelength ranges from 400 to 700 nm pour treating inflammatory processes, for instance on the skin and/or articulations of mammalians and human beings, in particular for curing arthritis, simple psoriasis, arthopathic psoriasis and neuropathies such as a carpal tunnel syndrome or Bechterev's disease.

In a method for non-therapeutic or therapeutic photodynamic treatment of a skin target area, a photosensitizer fluid incorporating a photosensitizing agent or a precursor for a photosensitizing agent is delivered to the target area, the photosensitizing agent having at least one absorption wavelength in a wavelength range higher than 400 nm. The target area is then exposed to light energy in a range of wavelengths comprising at least one absorption wavelength of the photosensitizing agent, the light energy being supplied to the target area in the form of at least one light pulse producing in the target area an accumulated energy density sufficient for photodynamic activation of the photosensitizing agent. The photosensitizing agent or precursor is incorporated in the photosensitizer fluid with a concentration in the range from 0.1 to 2.0 % preferably from 0.1 to 0.9 %, and the photosensitizer fluid is delivered to the target area in the form of a number of repeated spray doses during an overall delivery time ranging from 0.25 to 8 hours. The method can be applied to cosmetic treatments for skin rejuvenation, reduction of wrinkles, treatment of telangiectasia or removal of hair as wells a medical treatment of dermatologic diseases such as acne vulgaris, acne scars, rosacea, psorieasis, actinic keratoses, basal cell carcinoma, Bowen's disease or eczema.

The invention provides salts of 5-Aminolevulinic acid (ALA) of formula (I): wherein RY is an organic acid moiety; Y is selected from the group consisting of a sulfonic acid residue, mono- or di-phosphoric acid residue, mono- or di-carboxylic acid residue and R is selected from the group consisting of saturated, unsaturated, straight or branched C2-C20 chains, aryl, aralkyl or naphthyl. In preferred embodiments, RY is selected from benzenesulfonic acid (besylate), 2-naphthalene sulfonic acid (napsyate), p-toluenesulfonic acid (tosylate), diethyl phosphate, dibenzyl phosphate, di-(2-ethylhexyl) phosphate, caproic or stearic acids. The invention also provides methods for preparing the ALA salts of the invention, pharmaceutical compositions containing the ALA salts of the invention, and use of the ALA salts of the invention in photodynamic therapy (PDT).
