A system may receive signals indicative of medication dispensing activities by one or more health care providers such as nurses, physicians, or pharmacists. Based on the received signals, the system may determine one or more factor scores for each health care provider. The factor scores may be numerical indicators of potential diversion for corresponding factors related to usage, waste, dosage, or other factors. The factor scores may be combined to determine a total diversion score for each of one or more potential diverters.

The system includes a communications module configured to receive, from a client, a request for information regarding a medication dispense for a patient, and to obtain clinical information associated with the patient based on the request. The system also includes a processor configured to determine, based on the clinical information, whether a response to be provided to the client will include an alert regarding the medication dispense. The communications module is further configured to provide, to the client, the response to the request. Methods and machine-readable media are also described.

A system is provided for storage and reconstitution of IV solutions. The system may include one or more storage and reconstitution devices each configured to store and manipulate IV solution containers containing IV solutions in crystalline form that include hydration chemicals and therapeutic drugs. Crystalline hydration chemicals may include crystalline salts and sugars. The containers may be dimensionally precise enough for mechanical manipulation and configured such that sharp objects such as needles are not needed for fluid delivery to or from the container. Each storage and reconstitution device may store stacked containers and may include components for retrieval of a container from the storage and for reconstitution of the contents therein using a sterile water source within the device. IV doses may be manufactured, under appropriate standards, that can be mechanically reconstituted to produce high-quality doses in real time on the patient care unit or in the pharmacy.

Systems and methods of monitoring progress of delivery of a patient-specific medication are disclosed. A patient/medication identification (ID) device is provided on a package containing the medication, the patient/medication ID device comprising medication/patient information indicative of the medication and the patient. At least one location ID device is provided at a location, the at least one location ID device comprising a unique location ID associated with the location. The medication/patient information and the patient/medication ID are read. Delivery progress information indicative of a last-known read location where at least one of the medication/patient ID information and the unique location identifier was read is generated. The delivery progress information is stored in a database. The delivery progress information is accessed from the database in response to the request. A delivery status of the medication is indicated to the user.
