A method and apparatus for discriminating between stable and unstable ventricular tachycardias based on a measurement of oxygen saturation. Reference values for average oxygen saturation level and of the pulsatile characteristic of the oxygen saturation level are taken while a patient is in a resting condition. Measurements of oxygen saturation are also taken in response to the detection of a high heart rate, and are compared to the reference measurements to discriminate between stable and unstable ventricular tachyarrhythmias. An unstable tachyarrhythmia is diagnosed if a tachycardia is diagnosed and the current oxygen saturation level is similar to the reference average oxygen saturation level or if the oxygen saturation as presently measured displays a decreased pulsatile characteristic as compared to the reference measurements. The discriminator is intended for use in conjunction with or as part of an automated cardioverter of the type capable of delivering differing therapies for termination of stable and unstable ventricular tachyarrhythmias, such as antitachycardia pacing, cardioversion and defibrillation.

An apparatus for determining a respiration parameter in a medical device in which a pressure sensor senses pressure signals, and a signal processor, coupled to the pressure sensor, receives the sensed pressure signals and generates corresponding sample points. A microprocessor continuously adjusts a breath detection threshold in response to the generated sample points to generate a current adjusted breath detection threshold, compares a current generated sample point to the current adjusted breath detection threshold, suspends the continuous adjusting of the breath detection threshold and sets the breath detection threshold equal to the most current adjusted breath detection threshold generated prior to the suspending, and determines the respiration parameter in response to a comparing of a next generated sample point to the set breath detection threshold.

The onset of a syncopal episode is declared when the patient's respiration rate and/or tidal volume and/or minute ventilation increases by a predetermined increment or threshold and a heart rate drops below a threshold heart rate drop. The threshold heart rate drop is preferably established as a function of the change in the respiration parameter.

In one form the present invention relates to an external device for monitoring for sleep-related disordered breathing in communication with an implantable medical device for delivering an electrical stimulation therapy. In another form the present invention relates to an implantable medical device for detecting sleep-related disordered breathing episode(s) and an external apparatus (e.g., a CPAP machine) for providing therapy to terminate, and/or reduce, said episode(s). In this form of the invention, the implantable medical device communicates with the external apparatus so that the therapy provided corresponds in magnitude and duration to the severity and/or length of the episode(s). In yet another form, an implantable apparatus detects said disordered breathing episode(s) and a hybrid therapy is provided by both the implantable apparatus and an external apparatus.

An intracardiac ventricular pacemaker having a motion sensor is configured to produce a motion signal including an atrial systolic event and at least one ventricular diastolic event. The pacemaker is configured to set an atrial refractory period, detect a change in a ventricular diastolic event metric and adjust the atrial refractory period in response to detecting the change. If the pacing rate reaches the lower pacing rate without sensing intrinsic activity, heart rate drop detection remains disabled and lower rate pacing continues. Rate drop detection is re-enabled (530) whenever sufficient sensed intrinsic activity indicates that a sudden intrinsic rate drop could occur again.
