The oral appliance, either a mouthguard or an orthodontic device, includes a base member (2) having a U-shaped form corresponding to a row of teeth of a user. The base member (2) has inner and outer flanges (6, 7) interconnected by a web (5) which collectively define upper and lower channels (10, 11) within which respectively the upper and lower rows of teeth of the user are received. Upper and lower teeth engaging elements (3, 4) are positioned within the upper and lower channels (10, 11). The teeth engaging elements (3, 4) are made of EVA which softens at a temperature of 90-95 °C and are molded to the teeth of the user. The base member (2) remains unaffected by heating to 90-95 °C. The appliance may also include a tongue tag (25) and breathing apertures (30).

Methods and systems for monitoring biometric and/or environmental data of a user are provided. The user monitoring system includes a dental appliance configured to fit substantially inside a mouth of a user, a pulse oximeter for measuring user oxygen saturation level, a pressure sensor for measuring pressure exerted by the user and a processor for acquiring and processing oxygen saturation and pressure data of the user. The user monitoring system may communicate with an observer monitoring system. In some embodiments, the user monitoring system also measures one or more of ambient air temperature, light levels, respiration rate and environmental hazards.

The diagnosed soft tissue or brain injury component of concussions is generally defined by the symptoms of the temporal lobe manifestations. However, there is an undiagnosed structural fractured component of concussions occurring within the jaw joint complex which is the focus of this invention. A precise imaging technique and the powerful 3D Cone beam scanning technology, together, have revealed fractures in temporal bones of the jaw joint space which supports the temporal lobe of the brain. These fractures are the results of the lower jaw impact forces that cause concussions and this mechanism account for a large percent of the concussions arising in sports and military operations. These fractures have never been considered, diagnosed, or treated in the management of concussions. Fracturing temporal bones in the jaw joint that supports the temporal lobe will certainly produce the symptoms of temporal lobe manifestations. With early detection, these fractures can be more rapidly healed, eliminating many temporal lobe symptoms of concussions. The device of the present invention reduces the risk of the lower jaw impact concussion and temporal bone fractures, while enhancing the ability to speak and orally communicate.

The mouthguard of the present invention is provided with at least one recess in its base to receive a projection of a tool. The tool being made from a material which retains its shape at a temperature when the material of the mouthguard is ductile and susceptible to remoulding. By placing the mouthguard and tool together in the mouth of the user the thinning of the depth of the base of the mouthguard can be minimised whilst still enabling shaping of the mouthguard.
