    摘要: A post-surgical cardioverting system and method applies cardioverting electrical energy to the atria of the heart of a post-surgical heart patient. The system includes a first lead including a first elongated electrode having a proximal end and a distal end, a second lead including a second elongated electrode having a proximal end and a distal end, and sutures for releasably anchoring the distal ends of the first and second electrodes to the pericardium and disposing the first and second electrodes along the pericardium overlying the right and left atria respectively. A non-implantable cardiovertor is coupled to the first and second leads for applying the cardioverting electrical energy to the first and second electrodes. When cardioversion is no longer required, the first and second leads may be pulled out of the patient's chest.

    摘要: An implantable atrial defibrillator provides cardioverting electrical energy to the atria of a human heart in need of cardioversion. The atrial defibrillator includes a first detector for detecting ventricular activations of the heart, a second detector for detecting atrial activity of the heart, and an atrial fibrillation detector responsive to the second detector for determining when the atria of the heart are in need of cardioversion. The atrial defibrillator further includes a cardioverter for applying the cardioverting electrical energy to the atria of the heart when the atria of the heart are in need of cardioversion, and a timer delay stage responsive to the first detector for causing the cardioverter to apply the cardioverting electrical energy to the atria of the heart a predetermined delay time after the first detector detects one of the ventricular activations and before the T wave of the heart immediately following the one of the ventricular activations.

    摘要: An atrial defibrillator reliably detects a depolarization activation wave of the heart and delivers cardioverting electrical energy to the atria of the heart in synchronism with a detected depolarization activation wave. The atrial defibrillator includes a first sensor for producing a first electrical output having a duration substantially equal to the duration of the depolarization activation wave sensed between a first pair of spaced apart locations of the heart. The atrial defibrillator further includes a second sensor for producing a second electrical output having a duration substantially equal to the duration of the depolarization activation wave sensed between a second pair of spaced apart locations of the heart. The spacing between the second pair of locations is greater than the spacing between the first pair of locations. A microprocessor is responsive to the first and second electrical outputs for providing an indication signal indicating the occurrence of the depolarization activation wave when the second electrical output is longer in duration than the first electrical output. A pulse generator coupled to the microprocessor applies electrical cardioverting energy to the atria of the heart in response to the indication signal.

    摘要: An atrial cardiovertor/defibrillator provides therapy to the atria corresponding to the type of atrial arrhythmia occurring in the atria of the heart. The atrial cardiovertor/defibrillator includes a memory for storing respective different criteria for each of different types of atrial arrhythmia, a sensor for sensing activity of at least one of the atria of the heart to provide an electrogram signal, and a cardiovertor for providing a corresponding therapy to the heart for each of the different types of atrial arrhythmia. The cardiovertor/defibrillator further includes an atrial arrhythmia detector responsive to the electrogram signal and the stored criteria for identifying one of the types of atrial arrhythmia to cause the cardiovertor to provide therapy to the heart corresponding to the identified atrial arrhythmia.

A detector detects ventricular activations of the heart and a timer times intervals between immediately successive ventricular activations. The electrodes are electrically isolated from each other. The system further includes at least one anchor releasably disposing the electrodes beneath the skin of the patient with each atrial cardioverting electrode electrically contacting a respective given one of the atria, the atrial pacing electrode electrically contacting one of the atria, and the ventricular pacing electrode electrically contacting a ventricle. The system further includes at least one of an external cardiovertor coupled to the cardioverting electrodes for cardioversion, an external atrial pacer coupled to the atrial pacing electrode and one of the atrial cardioverting electrodes for pacing the atria, and an external ventricular pacer coupled to the ventricular pacing electrode and one of the atrial cardioverting electrodes for pacing the ventricles.
