    摘要: In a discectomy method one removes at least a substantial portion of a spinal disc. Thereafter one operates an ultrasonic instrument to level opposing faces of vertebrae on opposite sides of the removed spinal disc. Graft or synthetic disc material is inserted between the vertebrae so that the graft or synthetic disc material is in contact with the leveled opposing faces. In an associated nucleotomy, a cannulated probe is inserted into a spinal disc and used to remove the nucleus pulposus. A synthetic or substitute nucleus material may then be inserted into the evacuated annulus.

    摘要: A surgical instrument comprises, in accordance with the present invention, an elongate probe, a sleeve, a sleeve extension and at least one cauterization electrode. The probe is an ultrasonic element, serving to convey ultrasonic vibrations (as standing waves) to organic tissues at a surgical site. The probe has a working tip and a longitudinal axis. The sleeve surrounds the probe with the working tip of the probe projecting from a distal or free end of the sleeve. The sleeve extension is disposed at the distal or free end of the sleeve and defines a multiplicity of apertures having respective axes oriented at least substantially parallel to the longitudinal axis. The electrode has a distal or free end removably inserted through one of the apertures.

    摘要: An ultrasonic dissection and coagulation system for surgical use is provided. The system includes an ultrasonic instrument, a control module, and a pedal actuator. The ultrasonic actuator has a housing and an elongated body portion extending from the housing. An ultrasonic transducer supported within the housing is operatively connected to a cutting blade by a vibration coupler. The vibration coupler conducts high frequency vibration from the ultrasonic transducer to the cutting blade. The cutting blade has a cutting surface which is angled with respect to the longitudinal axis of the elongated body portion and, thus, with respect to the axis of vibration. A clamp member having a tissue contact surface is positioned adjacent to the blade member and is movable from an open position in which the tissue contact surface is spaced form the cutting surface to a clamped position in which the tissue contact surface is in close juxtaposed alignment with the cutting surface to clamp tissue therebetween. The clamp member and the angled blade combine to enhance contact between tissue and the cutting surface of the blade member.

In a disc space preparation procedure, the probe is inserted into a spinal disc between two vertebral endplates, ultrasonic vibratory energy is conducted into the probe, and the probe is manipulated to move the laterally enlarged head in three spatial directions within the spinal disc to thereby disrupt and shred the spinal disc material into fragments extracted from the space, in part by suction and in part by using forceps to pull spinal disc fragments from the operative space or surgical site between the vertebral endplates.

    摘要: An ultrasonic medical probe has a horn or shaft, a shank at a proximal end of the shaft, a probe head at a distal end of the horn or shaft, opposite the shank, and at least one polymeric component fixed to at least one of the horn or shaft, the shank, and the probe head. The shank is provided at a proximal end opposite the horn or shaft with an externally threaded connector for attaching the probe to a source of ultrasonic vibratory energy. The polymeric component is of a composition that transmits and is essentially impervious to ultrasonic vibratory energy but that degrades or decomposes upon exposure to a source of extreme energy (other than ultrasonic vibratory energy), rendering the probe inoperative for use.

    摘要: An ultrasonic surgical tool comprises a substantially planar blade body having a pair of opposed lateral surfaces and a cutting edge. A shank integral on a distal side with the blade body is provided at a proximal side with a connector for operatively linking the blade to a source of ultrasonic mechanical vibrations. The blade body is provided in at least one of the lateral surfaces with a shallow recess which is nearly coextensive with that lateral surface. The blade body has a raised rim surrounding and defining the recess, the rim being narrow on the three sides between the recess and the cutting edge.
