This invention provides a stereotactic device which is adapted for placement of a medical probe in the performance of percutaneous lumbar discectomy in a human body. The stereotactic device is utilized in conjunction with a fluoroscope for alignment and placement of a trocar type probe into a herniated disc. Bubble gauges are employed to level the x-axis and y-axis of a trocar support, which orients and slidably guides the medical probe into the targeted herniated disc nucleus.

A stereotactic device of simplified structure, adaptable to align and precisely orient a variety of medical devices, such as differently sized needles, cannulas, and guide wires, into the human body for procedures, such as tumor biopsies, percutaneous discectomies, cyst aspirations, and tumor localizations, is described. The stereotactic device is adapted for use in combination with a stereotactic bridge which includes a span rotatable on its horizontal axis and movably affixed to a C.T. scan table or an X-ray table by a positioning mechanism that provides for both vertical and horizontal movement of the span. The stereotactic device interacts with a C.T. scanner to achieve accurate placement of the selected medical device. Use of the stereotactic bridge allows alignment of the medical device in sagittal orientation with a high degree of accuracy. The stereotactic device is organized so as to allow a user to introduce accurately the medical device through a sterile field into the human body at a preselected angle. The stereotactic device includes a base, a leg normal to the base, a carrier for the medical device, and markings on the leg. The carrier for the medical device also has a radial portion, a retainer for the medical device, and a lock for fixing the medical device at a selected position.

A characteristic of optimal facial harmony is a correlation of the A/P (Anterior/Posterior) position of the maxillary incisors with the forehead. When the forehead is canted about 7 degrees or less, the A/P position of the maxillary incisors should match that of the middle of the forehead. For every degree the forehead is canted more than 7 degrees, the A/P position of the maxillary incisors should be 0.6 millimeters more anterior than the middle of the forehead. The extent to which the A/P position of the maxillary incisors is forward or back of the forehead's midpoint can be judged in millimeters quite accurately by viewing a patient in profile with the patient's forehead and maxillary incisors fully bared. Once the linear decision is made then the A/P treatment goal position can be physically measured and recorded using a novel bite fork (58), an improved facebow (56), and a new piece of equipment called a verticalizer (60). This information can then be transferred to and displayed on the occlusofacial simulator, a new kind of articulator.

A handheld guidance device for use in conjunction with a C.T. scanner allows a user to accurately place a catheter within a patient's body. The guidance device includes a generally planar base including a bubble level to aid in maintaining the base horizontal. A needle support arm is pivotally secured to the base adjacent one end thereof, and a cooperating protractor indicates the relative angular relationship between the needle support arm and the base. Needle guides are provided on the support arm for slidingly supporting the catheter at a desired angle as the catheter is inserted into the patient's body. Graduations are marked on the needle support arm for indicating the depth of insertion. A reference line formed upon the base is adapted to be aligned with a transverse light beam projected by the C.T. scanner.
