Abstract: An interatrial shunting device including a tube and an anchoring assembly. The tube defines a first end opposite a second end, and a tube wall extending to and between the first and second ends. The tube wall is a solid body and defines a lumen of the tube, with the lumen being open at the first and second ends. The anchoring assembly is carried by the tube and is configured to secure the interatrial shunting device to a native atrial septum. The solid wall tube prevents tissue overgrowth across the atrial septum, and can have minimal exposure of the device in the left atrium to minimize stroke and other risks. In some examples, the anchoring assembly is configured to be self-transitionable from a delivery state to a deployed state.

Abstract: An implantable medical device (IMD) including a fixation mechanism and a leadlet supporting an electrode. The leadlet includes a shape memory material configured to urge a leadlet body of the leadlet toward a preset orientation relative to a device body of the IMD. The leadlet is configured to establish a radial displacement between the device body and a distal end of the leadlet when the shape memory material urges the leadlet toward the preset orientation. The leadlet may be configured to cause the electrode to contact tissues of the heart when the fixation mechanism attaches to tissue of the heart and the shape memory material urges the leadlet toward the preset orientation.

Abstract: VfA cardiac therapy uses an implantable medical device or system. The implantable medical device includes a tissue-piercing electrode implanted in the basal and/or septal region of the left ventricular myocardium of the patient's heart from the triangle of Koch region of the right atrium through the right atrial endocardium and central fibrous body. The device may include a right atrial electrode, a right atrial motion detector, or both. The device may be implanted completely within the patient's heart or may use one or more leads to implant electrodes in the patient's heart. The device may be used to provide cardiac therapy, including single or multiple chamber pacing, atrioventricular synchronous pacing, asynchronous pacing, triggered pacing, cardiac resynchronization pacing, or tachycardia-related therapy. A separate medical device may be used to provide some functionality for cardiac therapy, such as sensing, pacing, or shock therapy.

Abstract: A fixation mechanism of an implantable medical device is formed by a plurality of tines fixedly mounted around a perimeter of a distal end of the device. Each tine may be said to include a first segment fixedly attached to the device, a second segment extending from the first segment, and a third segment, to which the second segment extends. When the device is loaded in a lumen of a delivery tool and a rounded free distal end of each tine engages a sidewall that defines the lumen, to hold the tines in a spring-loaded condition, the first segment of each tine, which has a spring-biased pre-formed curvature, becomes relatively straightened, and the third segment of each tine, which is terminated by the free distal end, extends away from the axis of the device at an acute angle in a range from about 45 degrees to about 75 degrees.

Abstract: Various fixation techniques for implantable medical device (IMDs) are described. In one example, an assembly comprises an IMD; and a set of active fixation tines attached to the IMD. The active fixation tines in the set are deployable from a spring-loaded position in which distal ends of the active fixation tines point away from the IMD to a hooked position in which the active fixation tines bend back towards the IMD. The active fixation tines are configured to secure the IMD to a patient tissue when deployed while the distal ends of the active fixation tines are positioned adjacent to the patient tissue.

Abstract: A catheter assembly includes a cap and a spring-biased tethering member coupled thereto. The cap includes first and second portions, and a transition zone extending therebetween. A girth of the first portion is sized to fit within a distal-most opening of the catheter assembly; and a girth of the second portion tapers from a first size at the transition zone, which is too large to fit within the distal-most opening, to a smaller size at a distal end of the cap. The spring-biased tethering member holds the cap in open and closed positions, when the cap first portion extends within the distal-most opening, and when the cap is separated from the distal-most opening, respectively. At the closed position, the first portion is approximately concentric with the distal-most opening, and at the open position, an entirety of the cap is laterally offset from the distal-most opening.

Abstract: In some examples, a system includes a catheter comprising an elongated shaft defining a lumen, and a tether assembly. The tether assembly may include an elongate body, a tether head assembly attached to a distal end of the elongate body, where the tether head assembly includes an attachment mechanism configured to releasably attach to an attachment member of a medical device, and a positioning element fixedly positioned over the elongate body and configured to align the attachment mechanism with the attachment member when the tether head assembly is extended distally out of the lumen. The positioning element may include a distal end, a proximal end, and a length between the distal end and proximal end that is less than a length of the elongate body. The tether head assembly, elongate body, and positioning element may be movable within the lumen of the elongated shaft.
