    摘要: Embolectomy catheters (10, 10', 10'', 10''', 10''''), rapid exchange microcatheters, systems and methods for removing clots or other obstructive material (e.g., thrombus, thromboemboli, embolic fragments of atherosclerotic plaque, foreign objects, etc.) from blood vessels. This invention is particularly useable for percuatneous removal of thromboemboli or other obstructive matter from small blood vessels of the brain, during an evolving stroke or period of cerebral ischemia. In some embodiments, the embolectomy catheters (10, 10', 10'', 10''', 10'''') of this invention are advanceable with or over a guidewire (GW) which has been pre-inserted through or around the clot. Also, in some embodiments, the embolectomy catheters include clot removal devices which are deployable from the catheter after the catheter has been advanced at least partially through the clot. The clot removal device may include a deployable wire nest (14) that is designed to prevent a blood clot from passing therethrough. The delivery catheter may include telescoping inner and outer tubes, with the clot removal device being radially constrained by the outer tube. Retraction of the outer tube removes the constraint on the clot removal device and permits it to expand to its deployed configuration. An infusion guidewire is particularly useful in conjunction with the embolectomy catheter, and permits infusion of medicants or visualization fluids distal to the clot.

    摘要: The present invention is a device and a method for boring a perfusion channel into the myocardial tissue of a patient. Structurally, the present invention includes a positioning catheter and a cutting catheter. The positioning catheter is formed with an inflation lumen and a deployment lumen and has an inflatable balloon mounted at its distal end. The cutting catheter has a sharpened distal end and is advanceable through the deployment lumen of the positioning catheter. Advancement of the cutting catheter through the deployment lumen causes the distal end of the cutting catheter to be directed radially outward from the positioning catheter. In use, the positioning catheter is positioned within a target vessel. The inflatable balloon is then expanded to anchor the distal end of the positioning catheter. The cutting catheter is then advanced through the deployment lumen, projecting the distal end of the cutting catheter radially outward from the positioning catheter, boring a perfusion channel in the tissue adjacent to the target vessel.

    摘要: A guidewire and guidewire assembly (10) for placement within a blood vessel for penetrating an occlusion therein. The guidewire comprises a length of flexible wire (12) having a concentric lumen (14) running its entire length and a distal end portion (20) having an arcuate tip (22) and a diameter greater than that of the wire immediately proximal thereto. The guidewire assembly comprises the guidewire described along with a flexible stylet (32) substantially the same length as the flexible wire and disposed within the lumen of the wire. In operation, the distal end portion is positioned in the blood vessel against an occlusion, and a dottering action is thereafter provided whereby the distal end portion repeatedly impinges on the occlusion until penetration of the occlusion occurs.

    摘要: Un fil de guidage (10) destiné à pénétrer dans une occlusion (62) se trouvant à l'intérieur d'un conduit ou d'un vaisseau, tel qu'un vaisseau sanguin, présente un corps de fil de guidage principal allongé (20) d'un premier diamètre réduit. Le fil de guidage se termine, en son extrémité distale, par une partie extrême souple élargie (24) présentant une surface extérieure lisse (22) et il est formé et construit pour pénétrer l'occlusion. Le corps du fil de guidage est construit pour être relativement souple dasn l'extrémité distale corrspondant au bout élargi, et relativement plus rigide dans ses parties proximales afin de permettre la manipulation du fil à partir de son extrémité proximale et afin de permettre à ladite partie extrême distale de pénétrer l'occlusion ainsi que d'y former un passage. Dans un mode de réalisation, un manchon extérieur (100, 206) glisse sur le corps principal, et venant en aboutement contre l'extrémité proximale de la partie extrême élargie il contribue à la rigidité axiale de la combinaison afin de permettre une manipulation à travers l'occlusion. Après la mise en place on retire ce manchon et l'on glisse un cathéter de dilatation (50) sur le corps principal, ledit cathéter pénétrant l'occlusion derrière la partie extrême élargie.
