    摘要: An emergency resuscitation and defibrillation apparatus (20) is provided by an endotracheal tube (22) having a tracheal obturator (30) and a second expandable cuff (32) for sealing against the pharyngeal tissues to provide an alternate sealing means for respiratory fluids if the blind intubation is not successful. A laryngeal tube (26) passes through this pharyngeal obturator (32) for alternatively introducing respiratory fluids into the lungs through the larynx (18). The endotracheal tube (22) may also be used as an esophageal obturator and inserted without the intubating guide means. Two internal electrode areas are provided; one of which (48) may be in the oral region for contacting the tongue and one of which may be in the esophagus (31,581 for contacting the esophageal wall. An internal conducting path is formed thereby for introducing electrical stimuli suitable for terminating cardiac fibrillation and other arrythmias. The endotracheal and laryngeal tubes may be individually accessed from outside the patient to ensure proper placement of the internal electrodes.

    摘要: An apparatus and method are provided for performing percutaneous or non-dissection tracheostomies. An outer needle (10) coaxially slidably receives an inner needle (20) which is attached to a syringe (24) adapted for use in inserting the needles (10,20) within the tracheal lumen (26). Operation of the syringe (24) confirms proper location of the needles (10,20) within the lumen (26). An elongated dilator (30) coaxially slidably mounts a trachea tube (42), and its proximal extremity constitutes an introducer portion (36) which tapers to a distal end (40) to which is attached an elongated, flexible leader (50) adapted for insertion through a bore (18) of the outer needle (10) subsequent to withdrawal of the inner syringe needle (20). A slot (16) in the outer needle (10) enables lateral separation of the leader (50) from the outer needle (10) to allow removal of the outer needle (10) from the trachea (26). The dilator (30) includes a recessed cutting edge (46) for cutting tissue compressed against it during forcible insertion of the introducer portion (36) into the tracheal lumen (26) along the path defined by the leader (50). The dilator (30) is slidably removable to leave the tracheal tube (42) in breathing position.

    摘要: An introducer for a tracheostomy tube is formed of several articulated sections (24) that can be changed from a relatively flexible state to a more rigid state by tightening a tension member extending along the introducer using an actuator at its machine end. In its flexible state the introducer can be inserted into and removed from the tube. The introducer is put in its more rigid state for use in inserting the tube into a tracheostomy. Alternatively or additionally the introducer could have several gripping rings on its outside towards its patient end and two elongate members that can be slid relative to one another to enlarge the patient end so that the gripping rings engage and grip the inside of the tube, thereby enabling the introducer to pull the tube through the tracheostomy from its patient end.
