    摘要: The invention contemplates apparatus and technique for surgically non-invasive surgery to remove cataracted-lens tissue, enabling the beam output of a laser to be focused from a convergentray bundle to a focul spot of maximum power density atthe anterior surface of a cataracted lens and which scans the focal spot over a predetermined area or areas of the cataracted lens. The beam is a focal and therefore diffuse as it enters the eye through the cornea, and it is also diffuse (being divergent) in the controllably unlikely event that the laser energy passes through an opening it has created in the cataracted lens; the diffusion assures against damage to either or both of the cornea and the retina, for power levels sufficient to achieve ablative photodecomposition and/or thermal decomposition and/or photofragmentation and/or photoemulsification of the cataracted-lenstissue. Various features are disclosed for assuring safety and uniformity in the removal of involved tissue.

    摘要: The invention contemplates use of a scanning laser characterized by ultraviolet radiation to achieve controlled ablative photodecompos i tion of one or more selected regions of a cornea. Irradiated flux density and exposure time are so controlled as to achieve desired depth of the ablation, which is a local sculpturing step, and the scanning action is coordinated to achieve desired ultimate surface change in the cornea. The scanning may be so controlled as to change the front surface of the cornea from a greater to a lesser spherical curvature, or from a lesser to a greater spherical curvature, thus effecting reduction in a myopic or in a hyperopic condition, without resort to a contact or other corrective auxiliary lens technique, in that the cornea becomes the corrective lens. The scanning may also be so controlled as to reduce astigmatism and to perform the precise incisions of a radial keratotomy. Still further, the scanning may be so controlled as to excise corneal tissue uniformly over a precisely controlled area of the cornea for precision accommodation of a corneal transplant.

    摘要: Disclosed is a completely portable, hand-held digital ultrasonic instrument about the size and shape of an ordinary pen. The housing is contoured such that it can be easily grasped in the manner of a writing pen. An activation button is located on the interior dorsal surface in close approximation to the index fingertip of the user. A liquid crystal display provides a digital readout of the corneal thickness on the barrel of the instrument housing. The instrument incorporates a 10 MHz or 20 MHz solid phase piezoelectric transducer, a microprocessor, a gate array, a hybrid analog circuit, a liquid crystal display, batteries and a removable battery cover. The 20 MHz transducer is attached to a swan neck connector which is attached to the instrument housing. When a 10 MHz transducer is used, it is attached to a truncated conical connector which is also attached to the instrument housing. In addition, a pinjack connector is avai ' able to link the unit to other electronic media such as a microcomputer, personal computer, or printer.

    摘要: A system for photoablating a photoablatable substrate (1) (such as the cornea of a human eye) to create a rounded work surface includes a curved, hollow mirror (5) for reflecting light capable of photoablating such surfaces. The curvature of the mirror determines the shape and curvature of the rounded work surface created. The mirror has an opening of sufficient size and shape to expose the substrate to reflected, photoablating light, and is linked to a mechanism (15) for adjusting mirror tilt and height with respect to the substrate. The system can include a source of photoablating light such as an excimer laser (8), a mechanism (7 and 4) for aligning light from the excimer laser with the mirror surface, a mirror cover (10) for excluding unreflected excimer laser light from desired portions of the substrate, and a high-speed shutter system (6) for controlling the amount and the timing of light transmission from the excimer laser (8) to the mirror (5).

    摘要: A microsurgery system comprising an instrument including a source of vacuum and control means, a remotely connected handpiece adapted to be disposed in the vicinity of the situs of the surgery and including an inlet and suction conduit means coupled to the inlet and to the vacuum source for carrying material away from the surgery s i tus. The system also includes a cassette assembly arranged for releasable securement to the instrument. The control means of the instrument includes an interrupter bar which cooperates with the cassette assembly for selectively isolating the vacuum source from the suction conduit means. The cassette assembly comprises a body portion, a collection vessel, a first port communicating with the suction conduit and a second port communicating with the vacuum source. Occludable conduit means are coupled between the first and second ports and are arranged to be engaged by the interrupter bar in response to a first signal to isolate the vacuum source from the suction means. The collection vessel is coupled to the suction conduit to collect the material extracted from the surgery situs. The cassette also includes similar means for enabling or disabling the flow of infusion liquid through an infusion line to the surgery situs. A hydrophobic filter and a check valve are also located within the cassette assembly.

    摘要: Eine Vorrichtung zur Operation an der Cornea des menschlichen Auges wird erfindungsgemäß aus verschie denen Instrumenten und Hilfsgeräten derart zusammenge setzt, daß die Transplantation eines scheibchenförmigen Teils der Cornea (perforierende Keratoplastik) weitestge hend maschinell-selbsttätig durchgeführt werden kann, wo bei mit der Hand ausgeführte chirurgische Manipulationen nahezu entfallen können. An einem dreidimensional ver stellbaren Träger sind ein Achseinstellinstrument (17), ein Trepan (18) und ein Transplantator (19) so angebracht, daß diese Instrumente wahlweise mit der Sehachse des Patien ten in Übereinstimmung gebracht und auf eine voreinstell bare Höhe verfahren werden können. Vorzugsweise umfaßt der Träger einen Hilfsträger (8) und einen an diesem um eine horizontale Achse (12) schwenkbaren Drehteller (11). Dieser trägt die Instrumente. An dem Hilfsträger (8) sind ein Bulbushaltering (14) höhenverstellbar und ein Manipulator (15) sowie ein Patronenwechsler (16) unbeweglich ange bracht. Die Instrumente und Hilfsvorrichtungen werden im einzelnen beschrieben. Die Vorrichtung insgesamt kann mittels einer Steuerkonsole (20) betätigt werden. Die Funk tionen und Programmabläufe sind elektrisch gesteuert, speicherbar und werden auf einem Monitor (21) sichtbar gemacht.
