摘要:
An apparatus for simultaneously applying to body tissue at least one surgical suture in conjunction with a plurality of surgical fasteners arranged end to end in rows. The apparatus preferably includes a knife for making an incision in the body tissue sealed in each side by at least one row of fasteners. The fasteners are two-part fasteners fabricated from bioabsorbable material. The incision is closed by pulling the ends of the suture(s).

An apparatus is disclosed for endoscopic application of surgical staples for fastening body tissue, especially vascular tissue. The apparatus includes a frame and an elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage cartridge cooperates with the distal end of the endoscopic section and is selectively pivotal by the user. A staple firing mechanism advances an individual staple against an anvil for staple closure. Means are provided to prevent incomplete staple firing. Relative motion between the staple cartridge and the anvil clamps tissue to be stapled therebetween. The endoscopic section may be adapted for rotation about the longitudinal axis of the apparatus. Gaseous seal means located in the endoscopic section prevent escape of insufflating gas through the apparatus.

A surgical instrument for placing fasteners and/or a reinforcement material in tissue is provided. Structure is provided for advancing the fasteners distally by an actuating handle means working in concert with a pusher means. The fasteners exit the fastener housing at an angle to the longitudinal axis of the device to facilitate visualization and placement at the surgical site. A unique fastener may be formed in which the legs are in a substantially overlapping longitudinally-spaced relation.

An apparatus for applying surgical clips through an endoscopic tube includes an endoscopic portion housing a jaw blade, a pusher bar for feeding clips one at a time to the jaws, a channel bar for camming the jaw blades into closed position, and an array of surgical clips. The body of the apparatus comprises a frame housing an actuating mechanism and a transmission mechanism for transferring movement to the pusher bar and channel. The endoscopic portion is rotatable relative Go the frame. In one embodiment the apparatus is a single disposable unit. In an alternative embodiment the endoscopic portion is detachable and disposable whereas the non-endoscopic body portion is reusable.

An apparatus is disclosed for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery. The apparatus includes a frame, and a generally elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage magazine is provided for storing a plurality of surgical staples at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. An elongated pusher system formed of several assembled components and extending from the frame to the endoscopic section is provided for individually advancing at least one staple at a time distally for positioning adjacent the surgical mesh and the body tissue. The pusher system also includes a trigger mecanism to actuate the pusher. Anvil means provides for closing the staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to the body tissue. The invention also relates to a system for attaching surgical mesh to body tissue, a method for endoscopically attaching surgical mesh to body tissue. The staple for endoscopically attaching the surgical mesh to body tissue and a kit for accomplishing such attachment.

The apparatus includes a handle and a generally elongated endoscopic section connected to the handle and extending distally therefrom. A staple storage magazine is provided for storing a plurality of surgical staples at the distal end portion of the endoscopic section. The staple storage magazine is pivotable between a first position in alignment with the longitudinal axis of the endoscopic section and a second position at an acute angle to the longitudinal axis. An elongated pusher system is provided for individually advancing at least one staple at a time distally. A trigger mechanism is provided to actuate the pusher.

A safety device is provided for use with endoscopic instrumentation, particularly and endoscopic clip applier. The device includes a collar which is adapted to be disposed on the elongated shaft of the endoscopic instrument. A release means is provided as part of the trocar guide sleeve which is adapted to contact the collar and release it from engagement with the elongated shaft.
