    摘要: In a surgical method, an elongate probe is inserted into a spinal disc exemplarily into a nucleus pulposus thereof in a direction generally parallel to vertebral end faces adjacent to the spinal disc. During the inserting of the elongate probe, the probe is ultrasonically vibrated. Thereafter, while ultrasonically vibrating the elongate probe, one moves the elongate probe to sever a prismatic portion of the spinal disc including a prismatic section of the nucleus pulposus. The prismatic portion of the spinal disc is removed from a remaining portion of the spinal disc.

    摘要: A discectomy method entails removing at least a portion of a spinal lamina to form an access path in a patient. A surgical instrument is inserted along the path and a distal end of the instrument is placed in operative contact with herniated or bulging disc material of a spinal disc. The instrument is operated to remove the herniated or bulging disc material, to thereby space a remaining portion of the spinal disc from spinal nerves. A tip of an ultrasonic surgical probe is then placed into contact with an outer surface of the remaining portion of the spinal disc, and an ultrasonic mechanical standing wave generated in the probe while maintaining the operative tip in contact with the outer surface to harden the wall surface and thereby reduce chances of disc herniation at the outer surface.

    摘要: An ultrasonic osteotome in accordance with the present invention comprises a shaft having a distal end portion with a lateral outer surface and further comprises a cutting blade connected to the distal end portion of the shaft at least in part at the lateral outer surface. The lateral outer surface extends so far on opposite sides of the blade as to block or stop penetration of the shaft into an incision formed in bone tissue by the blade. The blade can include a cutting notch or have a variable cutting depth depending on the angle of the distal end of the instrument to the surface of a target tissue mass.

    摘要: An ultrasonic surgical instrument includes an elongate substantially rigid probe and an elongate tubular sheath member. The probe is operatively connected at a proximal end to a source of ultrasonic mechanical vibrations and has a distal tip configured for transmitting ultrasonic vibration energy into organic tissues. The probe longitudinally traverses the sheath member. The sheath member has a distal edge with a first portion on one side of the probe and a second portion on an opposite side of the probe. The first portion of the distal edge of the sheath member is disposed substantially closer than the second portion of the sheath's distal edge to the distal tip of the probe. The second portion of the distal edge is so spaced from the distal tip as to permit effective visualization of the distal tip during use of the instrument.
