    摘要: Methods and apparatus for interrogation of implantable medical device (IMD) information, including programmed parameter values, operating modes and programmed changes thereof and data stored in the IMD, by emission of audible sounds by the IMD are disclosed. The IMD includes an audio transducer that emits audible sounds including voiced statements or musical tones stored in analog memory correlated to a programming or interrogation operating algorithm or to a warning trigger event. The audible sounds can comprise the sole uplink transmission or may augment the contemporaneous uplink RF transmission of stored data, and/or programmed operating modes and parameters and/or device operations and states in an interrogation or during programming. To conserve energy, the audible sounds accompanying interrogation and programming of the IMD are at a low volume that preferably cannot be heard without use of an external audio amplifier or stethoscope. Voiced statements warning of battery energy depletion or imminent delivery of a therapy are emitted at a higher volume that is sufficient to be heard by the patient, so that the patient can take appropriate action. The volume of audible sounds confirming programmed changes made by a patient using a limited function programmer or magnet is also increased so that the patient can hear them.

    摘要: A system and method for communicating with a medical device implanted in an ambulatory patient and for locating the patient in order to selectively monitor device function, alter device operating parameters and modes and provide emergency assistance to and communications with a patient. The implanted device includes a telemetry transceiver for communicating data and operating instructions between the implanted device and an external patient communications control device that is either worn by or located in proximity to the patient within the implanted device transceiving range. The control device preferably includes a communication link with a remote medical support network, a global positioning satellite receiver for receiving positioning data identifying the global position of the control device, and a patient activated link for permitting patient initiated personal communication with the medical support network. A system controller in the control device controls data and voice communications for selectively transmitting patient initiated personal communications and global positioning data to the medical support network, for initiating telemetry out of data and operating commands from the implanted device and transmission of the same to the medical support network, and for receiving and initiating re-programming of the implanted device operating modes and parameters in response to instructions received from the medical support network. The communications link between the medical support network and the patient communications control device may comprise a world wide satellite network, hard-wired telephone network, a cellular telephone network or other personal communications system.

    摘要: An improved method for manufacturing a sophisticated cam profile liner used in pumps and compressors. A non-circular cam profile liner is fabricated from multiple parts, preferably two identical cam liner halves mated together. Each cam profile liner half is generally crescent-shaped, and is manufactured from a single mold using conventional molding techniques. Window openings are formed in the cam profile liner without the need for hand-placing inserts in the mold. Further, no taper is formed in the cam profile liner. Accordingly, the present invention is suitable for use in precision sliding vane pumps, as well as flexible and roller-type pumps. Since each cam profile liner half is identical to the other, a symmetrical cam chamber is defined by the cam profile liner.

    摘要: A pacemaker capable of automatically adjusting the sensitivity of its sense amplifier to electrical cardiac signals is disclosed. In one embodiment, a pacemaker having a pressure sensor disposed on the distal end of its pacing/sensing lead counts the number of pressure events and electrical events which occur during an autosensitivity timing period. If the number of electrical events exceeds the number of pressure events by more than a predetermined margin, the sense amplifier's sensitivity threshold is decreased. If the number of electrical events does not exceed the number of pressure events by more than the predetermined margin, the sense amplifier's sensitivity threshold is increased. In another embodiment, the pacemaker maintains a running average of the peak voltages of sensed electrical events over a predetermined history period. Periodically, the pacemaker computes the ratio of the running average of peak voltages to a preprogrammed sense margin value, and adjusts the sensitivity threshold of the sense amplifier according to this computation. By basing the adjustment of the sense amplifier's sensitivity threshold on a long-term average of peak sense values, the effects of cycle-to-cycle variation in sensed values are minimized. In still another embodiment of the invention, a pressure sensor alone is employed to detect natural cardiac events.

    摘要: An implantable physiologic device, e.g., a multi-programmable, microprocessor based cardiac pacemaker, is provided with data storage and transmission capabilities for transmitting out certain current operating parameters and sensed events and for storing counted events for transmission of counts, histograms and real-time clock data out on command. The device preferably comprises a rate responsive cardiac pacemaker for providing an optimized pacing rate of stimulation pulses as a function of at least one selected rate control parameter. Each rate control parameter has a value which varies as a function of changes in a patient's physiologic demand and includes a sensor system for sensing the rate control parameter value and for providing a sensor output representative thereof. The cardiac pacemaker also includes control circuitry which includes a rate response defining means for deriving desired pacing rates as a function of the sensor output signal(s) modified by sensor gain and selective weighting coefficient values. An achievement monitoring means that has a predetermined achievement criterion monitors the relationship between the derived pacing rates and the achievement criterion over an optimization period. Output circuitry providing optimized pacing rates as a function of optimization is accomplished by adjusting the rate response gain for each sensor as a function of the monitored achievement relationship. Another optimization function is provided by adjusting a sensor weighting coefficient value which weights or regulates the relative contribution each sensor's derived desired pacing rates will contribute toward the pacemaker-derived optimized pacing rate.

    摘要: An implantable heart pacemaker having digital injection integrated circuitry includes a bias switch (70) operative to turn on or increase the bias current to the pulse width logic (50) just prior to an output pulse. Increased bias to the pulse width logic permits high speed operation of the logic to accurately terminate the output pulse at the end of the desired pulse duration. At the end of the output pulse, the bias switch is turned off to return the bias current to a low quiescent value, and in some cases is turned completely off, to conserve battery power. The logic gates used in the pulse width logic are arrayed along one or more current injector "bars" or "rails" formed in the integrated circuit, and the bias switch (70) is connected for controlling the current delivered to those current injectors, so that increased bias need only be supplied to the circuits requiring high switching speed.
