摘要:
The general purpose of the present invention is to provide a catheter for removal of an organized embolic thrombus, preferably in conjunction with a distal occlusion balloon guidewire. The present invention consists of three main components including a capture/delivery sheath, a capture sleeve of mesh attached to a tube and a grasping mechanism attached to another tube which is central to the other components. Operative structures in the form of manifolds are connected to such components in order to manipulate and control the relative positions of the capture/delivery sheath, the capture sleeve and the grasping mechanism. An embolic thrombus is engaged by the deployable automatically expanding serrated fingers of a grasping mechanism in combination with a deployable automatically expanding mesh capture sleeve. The capture/delivery sheath is maneuvered to cause compression of the serrated fingers and of the mesh capture sleeve for subsequent removal of the embolic thrombus.

A miniature cross stream thrombectomy catheter useful in small blood vessel thrombectomy procedures. An annulus near the distal end of the miniature cross stream thrombectomy catheter is formed between a hypo-tube and a proximally directed bore of a concentrically aligned flow director whereby jet orifices in the hypo-tube communicate with the annulus to direct jet flows of saline or other fluid around and about the annulus and to be directed proximally from the annulus to pass through one or more outflow orifices and thence to pass through one or more inflow orifices thereby creating an ablation flow therebetween which loosens and carries away thrombotic materials from the walls of a blood vessel.

A single operator exchange fluid jet thrombectomy method employing a single operator exchange fluid jet thrombectomy device having an outer catheter assembly and separable and exchangeable components in the form of an inner catheter assembly allowing functioning as a rheolytic thrombectomy catheter or as a crossflow thrombectomy catheter. The outer catheter assembly is common to any mode of usage and includes a guide catheter having a lumen through which a guidewire and the greater portion of a hypo-tube carrying a jet emanator and a flow director are passed and advanced. In the method, thrombus is dislodged, entrained, and broken into pieces by fluid jets and evacuated through the lumen of the guide catheter.

A single operator exchange fluid jet thrombectomy device having an outer catheter assembly and separable and exchangeable components in the form of an inner catheter assembly allowing functioning as a rheolytic thrombectomy catheter or as a crossflow thrombectomy catheter. Embodiments include an outer catheter assembly common to any mode of usage having a guide catheter having a lumen through which a guidewire and the greater portion of a hypo-tube carrying a flow director are passed and advanced. For use as a rheolytic thrombectomy catheter, thrombus is dislodged, entrained, and broken into pieces by jets and evacuated through the lumen of the guide catheter. For use as a crossflow thrombectomy catheter, a flow director having outflow and inflow orifices is provided.

A balloon catheter (10) having a shaft (16), a balloon (14), and an external delivery tube (12) is disclosed. Methods of using the balloon catheter having an external delivery tube are also disclosed. In one example, the balloon catheter is an agent delivery catheter including a catheter body extending between catheter proximal and distal portions. An inflatable balloon assembly (100) is coupled with the catheter body. An agent delivery assembly (102) is coupled with the catheter body and the inflatable balloon assembly. The agent delivery assembly includes a delivery lumen (32) extending through the catheter body, and an agent delivery tube extending along an exterior balloon surface (64). The agent delivery tube includes at least one delivery orifice (70) directed outside of the exterior balloon surface. The agent delivery assembly is isolated from fluid communication with the inflatable balloon assembly.

A thrombectomy catheter with a self-inflating proximal balloon having drug infusion capabilities is described. A self-inflating balloon is formed from an inflatable thin walled section of a flexible catheter tube. The self-inflating balloon includes a plurality of outflow orifices located about the peripheral circumference thereof and located proximal to an inflow gap interposed between a fluid jet emanator and the self-inflating balloon. The self-inflating balloon is inflated and expanded by internal operating pressures by proximal composite flow of fluid from the fluid jet emanator and entrained fluid from the inflow gap to uniformly space and position the outflow orifices of the self-inflating balloon in close proximity to the thrombus or vessel walls of a blood vessel. The thrombectomy catheter may be used for, among other things, thrombectomies, embolectomies, thrombus or vessel dilation, and for the delivery of drugs to a thrombus or vessel site.

An enhanced cross stream mechanical thrombectomy catheter having a distal end with outflow and inflow orifices at one side for producing concentrated cross stream jet flow for selective and concentrated thrombus ablation during thrombectomy procedures. A multidirectional fluid jet emanator positioned within the distal catheter lumen comprises high velocity fluid jets which produce jet flow that is directed toward distal and proximal ends of the catheter. Jet flow directed toward the proximal end provides a region of low pressure near an inflow orifice which acts to pull thrombotic particulate from the vasculature. The invention provides for suitable distancing of high powered ablation or suction forces from the near walls of the vasculature. Cross stream flow emanating from one side of the catheter distal end urges the opposing non-orificed side of the catheter distal end toward and against the vascular wall to inhibit contact of the inflow orifice with the vascular wall.

An enhanced cross stream mechanical thrombectomy catheter with backloading manifold having a distal end with outflow and inflow orifices at one side for producing concentrated cross stream jet flow for selective and concentrated thrombus ablation during thrombectomy procedures. The invention provides for suitable distancing of high powered ablation or suction forces from the near walls of the vasculature. Cross stream flow emanating from the one side of the distal end of the catheter resultantly urges the distal end of the catheter and thus the opposing non-orificed side of the distal end of the catheter toward and against the vascular wall to inhibit contact of the inflow orifice with the vascular wall. Features of the invention include a geometrically configured insert to facilitate the backloading or exchange of guidewires.

The present disclosure involves a rheolytic thrombectomy catheter with a self-inflating proximal balloon having drug infusion capabilities. A self-inflating balloon having a plurality of outflow orifices located about the peripheral circumference thereof is located proximal to an inflow gap or orifices and a fluid jet emanator and which balloon is inflated and expanded by internal operating pressures to uniformly space and position the outflow orifices of the self-inflating balloon in close proximity to or in intimate contact with the thrombus or walls of the blood vessel. Devices of the present disclosure can be used for the purposes of thrombectomies, embolectomies, thrombus or vessel dilation and for the delivery of drugs to a thrombus or vessel site.
