    摘要: A cardioversion/defibrillation device includes a lead catheter housing, two electrically independent conductors, and a pair of resilient helically coiled electrodes, one electrode coupled to each of the conductors. The distal ends of the electrodes are contained within an insulative, resilient coupling sleeve, such that the electrodes and sleeve form a loop when in the relaxed state, with the electrodes electrically isolated from one another. To facilitate body insertion and delivery to the electrode placement site, the electrodes and coupling sleeve are loaded into a delivery cannula, which elastically deforms the electrodes and sleeve into an elongate, narrow delivery configuration. Upon release from the delivery device, these components resiliently return to the loop configuration. If desired, an inextensible filament secured to the coupling sleeve is pulled proximally after deployment to selectively reconfigure the electrodes into two loops. Another modification involves several linear electrode branch segments, coupled to each electrode and projected inwardly of the loop or loops formed by the electrodes.

    摘要: A defibrillation electrode for implantation on or about the heart and for connection to a defibrillation system. The electrode comprises a plurality of electrically conductive elements spaced apart and electrically connected together, thus, increasing the number of discharging edges on the electrode. In a first embodiment, the electrode comprises a plurality of concentric conductive rings electrically connected together. In a second embodiment, the electrode comprises a plurality of conductive planar elements electrically connected together in a generally puzzle-like configuration. In a third embodiment, the electrode comprises electrically conductive wires wrapped around the length of a cardiac catheter. In a fourth embodiment, electrically conductive wires are concentrically spiralled into a spiral patch configuration. In a fifth specific embodiment, a plurality of electrically isolated active sites are provided on the distal portion of an endocardial catheter. In all of the above embodiments, the electrical discharge of energy from the electrode to the heart surface favors the conductive edges of the electrically conductive and connected components of the electrodes. Therefore, by increasing the number of conductive edges, a more uniform distribution of energy is delivered to the heart. Furthermore, each of the electrode configurations may be constructed without an insulative backing to further increase the efficiency of discharge.

    摘要: A bipolar lead for use in sensing cardiac depolarization signals and for applying electrical stimulation to cardiac tissue comprises a pair of elongated conductors contained within a lead body, the lead body terminating at its distal end in an enlarged head fabricated from an insulating plastic material. The head has a generally planar surface and projecting perpendicularly from that surface is a corkscrew electrode which is adapted to be screwed into myocardial tissue. Also supported on the planar surface of the head so as to surround the corkscrew electrode is a toroidal electrode formed from helically wound conductive wire. One of the conductors passing through the lead body connects at its distal end to the corkscrew electrode which the other conductor attached to the toroidal electrode. The toroidal configuration of the helically wound wire provides substantially increased surface area when compared to a flat disk electrode and, moreover, offers the ability of tissue to grow into the interior regions of the toroidal electrode for improved anchoring, higher amplitude of the sense depolarization signals and a higher slew rate for the signal. Another feature of the invention involves filling the interior of the hollow torus structure with a wool-like material formed from platinum or other inert conductive material for even further increasing the amount of electrode surface in contact with tissue.

    摘要: An implantable apparatus for continuous or periodic introduction of a drug to a desired release point in the body comprises an implantable power source and an elongated flexible plastic tubular catheter having a lumen filled with the drug to be delivered and containing therein a first device electrode. The distal end of the catheter is closed and located near the distal end is a port which extends through the wall of the catheter to communicate with the lumen. Just proximal of the port is a surface electrode mounted on the exterior wall of the catheter body. Extending through the catheter and joined to the drive electrode and the surface electrode are conductors which couple those electrodes to the implanted power source. Energization of the electrodes with a predetermined direct current potential causes a flow of the ionized drug molecules out through the port and into the area surrounding the surface electrode.

    摘要: An implantable cardiac pacing lead includes a porous, platinum electrode, a flexible electrically conductive coil, and a crimp tube coupling the electrode to the distal end of the coil. A recess in the crimp tube, open to the electrode at the crimp tube distal end, houses a matrix impregnated with a therapeutic drug. The electrode is highly porous and loaded with a therapeutic drug in liquid or solid form. Immediately upon implant of the electrode at a selected myocardial location, the electrode begins dispensing the therapeutic drug. Meanwhile, the matrix begins to elute its drug at a rate more suited to chronic treatment. If desired, the recess can extend through the crimp tube to permit proximal end loading of the drug carrying matrix. A silicone plug, or an additional and proximal crimp tube section, is inserted into the recess after loading of the matrix to provide the required fluid seal. Finally, a variety of different matrices can be housed in the recess, to provide elution of different drugs and at differing rates.

    摘要: Various aspects of the present subject matter provide an implantable medical device. In various embodiments, the device comprises a pulse generator, a lead, a sensor, and a controller. The pulse generator generates a baroreflex stimulation signal as part of a baroreflex therapy. The lead is adapted to be electrically connected to the pulse generator and to be intravascularly fed into a heart. The lead includes an electrode to be positioned in or proximate to the heart to deliver the baroreflex signal to a baroreceptor region in or proximate to the heart. The sensor senses a physiological parameter regarding an efficacy of the baroreflex therapy and provides a signal indicative of the efficacy. The controller is connected to the pulse generator to control the baroreflex stimulation signal and to the sensor to receive the signal indicative of the efficacy of the baroreflex therapy.
