    摘要: Disclosed herein are high efficiency surgical devices and methods of using same using radio frequency (RF) electrical power to destroy, vaporize and remove soft tissues, such as tumors, both malignant and benign, from within a target surgical site. In one particularly preferred embodiment, the electrosurgical device employs a combination of rotary and translational motion to incrementally vaporize a calculated volume of tissue. According to the principles of this invention, the electrosurgical devices can be used with externally supplied conductive or non-conductive irrigants, whether liquid, gas, or a combination thereof, as well as without externally supplied liquids, a mode of operation often referred to as “dry field” environment. The electrosurgical devices may further optionally include aspiration components to permit removal of vaporization by-products.

    摘要: A suture punch system is capable of directly passing braided suture through tissue in a simple, one-step process. The system includes three principle components: a malleable needle with braided suture attached, a handheld instrument for grasping tissue and controlling needle placement, and a trocar to supply the force required for needle placement. Needle deformation begins at the tip of the instrument, which preferably includes a curved segment. As the distal tip of the needle pierces the tissue, it continues its radial path through the tissue. When the proximal end of the needle exits from the instrument, the needle is entirely radial in shape and traverses an essentially radial path through the tissue. Non-radial linear or non-linear segments may alternatively be used. In one embodiment, the tissue to be sutured is constrained by pressure applied through closure between an upper, moveable jaw the distal portion of the instrument which acts as a fixed jaw. The upper, movable jaw contains a shaped passageway which allows the curved needle to pass therethrough during use. Once the needle has passed entirely through the tissue it is ready for retrieval using the jaws of the punch or another instrument. Different needle designs and separate instruments are also disclosed.

An electrode having a rectangular ablation surface formed of multiple parallel ribs or protuberances is assembled onto the mandrel so that the ablation surface is parallel with the longitudinal axis of the mandrel. A ceramic insulator provided with an opening of rectangular shape is assembled to the electrode-mandrel assembly. The insulator is further provided with a cylindrical passage in communication and aligned with a radial passage of the mandrel.

    摘要: A process for simultaneously shaping and sharpening a cutting widow at the distal end of a tubular member of a rotary blade assembly. The assembly is provided with a stationary elongated outer tube, having a cutting window at its distal tip and includes a rotatable elongated inner tube having a cutting means at its distal tip. The window in the outer tube has a curvilinear profile defined by a peripheral rim surrounded entirely by a land surface which is inclined relative to the rim. The inclination of the land produces an area which tapers from a full thickness, where the land is adjacent to the cylindrical wall of the outer tube, to a sharp edge around the periphery of the window. The curvilinear window is produced by a manufacturing process which creates the window opening with a sharpened periphery at the same time that the land surrounding the opening is inclined. A preferred embodiment utilizes electrochemical grinding of the distal ends of the tubular member by a tool having a rotatable wheel with a perimeter in the shape of a groove of predetermined arcuate profile.

    摘要: A shaver blade assembly useable in either an irrigation-only mode, an aspiration-only mode or an irrigation/aspiration mode. The shaver blade assembly has a stationary elongated outer tube and a movable (e.g. rotatable) elongated inner tube, both inner and outer tubes having hubs attached to their proximal ends for attachment to a handpiece which provides power to move the inner blade relative to the outer blade. The inner and outer tubes are provided with cutting windows at their distal tips and the relative movement between these cutting windows acts to cut tissue during surgical procedures. The outer tube is provided with a fluid inlet port at the proximal end of its tubular surface and a fluid adapter is selectively attachable to the outer tube so as to provide a means for introducing irrigating fluid into the fluid port. The adapter is integrally formed with sealing surfaces which obviate the need for O-rings and the like. A longitudinally extending irrigating channel is provided between the inlet fluid port at one end of the irrigating channel and the outlet fluid port at the distal end of the irrigating channel by which fluid is communicated to the vicinity of the cutting windows when the shaver blade is used in the irrigation-only mode or the irrigation/ aspiration mode. The channel may be either the annular clearance space itself or a separate, dedicated groove or channel formed in the mating surfaces of the tubular members. Removing the adapter and the fluid supply enables the shaver to continue being used in an aspiration-only mode.
