    摘要: A method and device provides features for the temporary “parking” of sutures with respect to a cannula. In the preferred embodiments the features are slots that maintain tension on, and orientation of, sutures placed therein. The slots or features may be integral with the cannula or part of a separate device which may be removably affixed to the external portion of a cannula. In addition, the opening(s) used for instrument-passing are off-center, thereby reducing if not elimination outward sprays during use. The invention eliminates the multiple steps inherent in current suture management techniques for arthroscopic procedures, particularly rotator cuff repair and assists in tensioning/advancement of tissue into its repair site.

    摘要: A suture punch system that is capable of directly passing braided suture through tissue in a simple, one-step process. The system includes three principle components: a malleable needle capable of delivering the suture to the tissue, a handheld instrument for grasping tissue and controlling needle placement, and a force-supplying mechanism to supply the force required for needle placement. Needle deformation begins at the tip of the instrument, which beneficially includes a curved segment. As the distal tip of the needle pierces the tissue, it continues its radial path through the tissue. When the proximal end of the needle exits from the instrument, the needle may be radial in shape and traverses an essentially radial path through the tissue. Once the needle has passed entirely through the tissue, it may be retrieved using the jaws of the punch or another instrument.

    摘要: A suture punch system is capable of directly passing braided suture through tissue in a simple, one-step process. The system includes three principle components: a malleable needle with braided suture attached, a handheld instrument for grasping tissue and controlling needle placement, and a trocar to supply the force required for needle placement. Needle deformation begins at the tip of the instrument, which preferably includes a curved segment. As the distal tip of the needle pierces the tissue, it continues its radial path through the tissue. When the proximal end of the needle exits from the instrument, the needle is entirely radial in shape and traverses an essentially radial path through the tissue. Non-radial linear or non-linear segments may alternatively be used. In one embodiment, the tissue to be sutured is constrained by pressure applied through closure between an upper, moveable jaw the distal portion of the instrument which acts as a fixed jaw. The upper, movable jaw contains a shaped passageway which allows the curved needle to pass therethrough during use. Once the needle has passed entirely through the tissue it is ready for retrieval using the jaws of the punch or another instrument. Different needle designs and separate instruments are also disclosed.

    摘要: Disclosed herein is a distal end electrode assembly for use in connection with electrosurgical devices, particularly those adapted for the modification, sculpting, resection, removal, or vaporization of tissue, configured for coagulation, cauterization or hemostasis purposes, or utilized for thermal treatment of normal and tumorous tissues. In the context of the present invention, mechanical fastening means, epoxies and other high-temperature adhesives connecting electrode(s) to insulator(s) are replaced with brazed joints to yield electrosurgical devices capable of safely and reliably operating at high power densities and elevated temperatures without thermal failure of the bonds between the electrode and the insulator. The use of brazed joints further permits the construction of miniaturized, compact electrosurgical devices, of both monopolar and bipolar configurations, having utility in a number of divergent fields, from arthroscopy to otolaryngology to oncology, and applicable to both laparoscopic and open surgery techniques. Thus, active electrodes and electrosurgical devices of the present invention maximize efficiency, safety and reliability while minimizing manufacturing cost and device profile.
