摘要:
A stent delivery system having an expandable stent in a contracted condition positioned on an expandable member, such as an inflatable balloon, disposed on the distal portion of an elongated catheter body. A guidewire extends through an inner lumen within the elongated catheter body and out its distal end. A tubular protective sheath is secured by its distal end to the portion of the guidewire which extends out of the distal end of the catheter body and fits over the rolled up stent mounted on the expandable member on the distal end of the catheter body. The catheter assembly is advanced to a desired location within a patient's body lumen, such as the patient's arterial system, with the sheath in place over the stent. Once in proper position, the guidewire is advanced distally in order to remove the attached sheath and completely expose the stent. The expandable member on the catheter is expanded to expand the stent. The stent is preferably self-locking in the expanded condition so that the expanded expandable member can be contracted and the catheter can be removed therefrom.

A gastric band assembly has one or more bladders incorporated therein so that basal intra-band pressure in the band can be set higher than typical Green Zone basal intra-band pressures. The higher basal intra-band pressures equate to higher basal intra-luminal or contact pressures from 35 mmHg and above.

A gastric band assembly has one or more bladders incorporated therein to minimize or modulate changes in intra-band pressure in response to changes in stoma area and band stoma area. The balloon portion of the gastric band encircles stomach tissue thereby forming a band stoma area. With the bladders incorporated in the gastric band assembly, the affect that changes in band stoma area have on intra-band pressure are minimized so that the patient stays at or near the physician set intra-band pressure longer than with a gastric band alone.

A bladder assembly is provided in order to maintain the pressure in the balloon portion of a gastric band in a range corresponding to a so-called Green Zone. Multiple bladders are connected by flexible tubing which is connected at a distal end to the balloon portion of a gastric band. The elastically expandable bladders provide fluid pressure on the balloon portion of the gastric band in order to maintain the intra-luminal pressure within a desired range over a prescribed fill volume.

The invention relates to an endoluminal prosthesis adapted for placement at a bifurcation site within the body. The stent or stent-graft may be constructed to have segments of differing structural properties. A section of the stent-graft may be constructed to have a single-lumen tubular stent member covering a multilumen graft member. The stent-graft may comprise at least two modular components adapted for in situ assembly. An extended cylindrical interference fit may be used to seal the modular components.

A self-expanding, self-restraining endoluminal prosthesis is configured to be compressible into a collapsed state, and restrained in the collapsed state by structures integrated with the stent, and without the use of external structure. Such restraining structures engage one another to hold the prosthesis in the collapsed state. Expansile elements of the prosthesis urge the prosthesis to automatically expand once released from the collapsed state. Thus, once the restraining structures, which hold the prosthesis in the collapsed state, are disengaged, the prosthesis automatically expands to a deployed state.

A cardiac harness is configured to fit about a portion of a patient's heart so as to exert a compressive force on the heart during at least a portion of the cardiac cycle. The harness can be constructed of a plurality of individual modules assembled ex vivo or in vivo. The modules can have different physical characteristics, such as having different compliance, and may or may not include spring hinges. Portions of a cardiac harness can be connected to each other using a coupling mechanism such as, for example, a zip coupler.

A cardiac harness for treating congestive heart failure is disclosed. The harness applies elastic, compressive reinforcement on the left ventricle to reduce deleterious wall tension and to resist shape change of the ventricle during the mechanical cardiac cycle. Rather than imposing a dimension beyond which the heart cannot expand, the harness provides no hard limit over the range of diastolic expansion of the ventricle. Instead, the harness follows the contour of the heart throughout diastole and continuously exerts gentle resistance to stretch. Also disclosed is a method of delivering the cardiac harness to the heart minimally invasively.

A delivery system for delivering and mounting a cardiac harness having electrodes on a heart including an elongate body having a proximal portion and a distal portion. The body includes a cavity sized to contain the cardiac harness and electrodes in a compacted configuration and also includes a plurality of elongate push rods movable with respect to the body. The cardiac harness and electrodes are releasably connected to the push rods such that, advancement of the push rods in a distal direction moves the cardiac harness and electrodes from a compacted configuration, within the cavity, to an expanded configuration, outside the cavity. The apparatus also includes a releasing member for releasing the connections between the push rods and the cardiac harness and electrodes upon actuation of the releasing member by a user.
