摘要： A patient healthcare and monitoring system facilitates communications between doctors and patients. The system includes a server hosting a database containing patient information including patient medical histories. Physicians are provided with a doctor client in the form of, for example, a Tablet PC with which a physician can set up patient alerts, surveys and messages and receive reports, messages and patient history. Patients are provided with a patient client in the form of, for example, a Smart phone, Pocket PC or PDA with which a patient receives alerts, messages and surveys generated by their physician and transmit alert completions, messages and survey answers. The doctor clients and the patient clients communicate with the server so that patients are alerted when to perform prescribed procedures or take prescribed medications and physicians are informed that prescribed procedures have been performed or medications taken and receive answers to surveys which become part of a patient's medical history.
