Abstract: A method of providing all or portions of a care program for a patient includes receiving data regarding the patient from a number of sources. At least some of the data is then used in determining a virtual readiness profile for the patient. A number of care elements for the patient are then determined based on the patient's virtual readiness profile and then provided to the patient and/or a caregiver of the patient.

Abstract: A mechanism for estimating the relevance or importance of sections of unstructured (medical) textual data. Different sections are visually emphasized responsive to a user input, and one or more characteristics of when the sections are visually emphasized are monitored. Other characteristics of the sections may also be monitored or determined. These characteristics are then processed to predict or determine a relevance of each section.

Abstract: The present disclosure is directed to methods and systems for predicting patient dropout from a remote patient monitoring (RPM) program, as well as root dropout causes, based on clinical features using a “dropout prediction engine”. As described herein, the methods and systems address the clinical challenge of early detection of dropout risk of patients from these virtual care programs through a data-driven approach that accurately identifies the likely root cause(s) of the dropout and enables the prevention of the dropout by applying timely interventions targeting the root causes of the dropout. As a result, dropout prevention effectuated through targeted interventions will promote continued engagement with virtual care, thereby leading to lower costs of care, better health outcomes, and better patient and staff experience.

Abstract: The present disclosure is directed to systems and methods of establishing a patient care network that enables high-quality and safe involvement of selected caregivers in the care of patients receiving virtual or at-home care. As described herein, the methods comprise: identifying a first informal care recipient; identifying an informal caregiver candidate based on caregiver information; assigning the informal caregiver candidate to the first informal care recipient; evaluating a performance of the informal caregiver candidate; determining a certification classification/information for the informal caregiver candidate; and creating and/or storing a caregiver record of the informal caregiver candidate in an informal care database.
