    Abstract: A tether (10) can be transluminally advanced into a coronary blood vessel of the heart. At a first location (14a), a first terminus (10a) of the tether is advanced through a wall of the vessel into a chamber of the heart, and is then transluminally drawn out of the chamber such that a first segment of the tether extends from the first location through the chamber. At a second location (14b), a second terminus (10b) of the tether is advanced through the wall, into the chamber, and is drawn transluminally out of the chamber such that a second segment of the tether extends from the second location through the chamber. At least one anchor (50) is slid over and along at least one of the first and second termini and anchored to tissue of the chamber. The chamber can then be reshaped by modifying tension in the tether.

    Abstract: Mitral valve prolapse and mitral regurgitation can be treating by implanting in the mitral annulus a transvalvular implant. The transvalvular implant has a first end, a first anchoring portion located proximate the first end, a second end, a second anchoring portion located proximate the second end, and a central portion. The transvalvular bridge can be positioned between a midpoint of the anterior annulus and straddling the commissure between the posterior annulus and the septal annulus. The transvalvular bridge can be anchored to the anterior annulus, the posterior annulus, and the septal annulus.

    Abstract: An embolic filter positioning system includes a vascular delivery sheath, a filter catheter, a self-expanding embolic filter attached to a distal end of the filter catheter, and an elongate dilator having a tapered dilator tip. The elongate dilator is slidably received in a central lumen of the filter catheter, and the filter catheter is slidably received in the open lumen of the vascular delivery sheath. The tapered dilator tip is positioned distally of the distal end of the vascular delivery sheath, and the self-expanding embolic filter is radially constrained in a proximal portion of the open lumen of the vascular delivery sheath. The tapered dilator tip has an expanded configuration where it covers the open distal end of the vascular delivery sheath and facilitates entry though an arteriotomy and a contracted configuration where it can be retracted through the central lumen of the filter catheter..
