Various forms of a cable fixation device, instrumentation, kit, and methods useful for repairing the skeletal system are introduced. The system utilizes a clamp housing fixing a head end of a surgical cable therein. In an operative configuration the cable is looped around a damaged bone segment and reentered through a lock aperture in the clamp housing then through a multi-part collet and lock cap residing within a lock aperture. The cable loop and each aforementioned component comprise a central axis aligned within a single plane. A sliding interface situated between the lock cap and collet prevent twisting of the surgical cable. The locking mechanism is non-destructive to the cable despite repeated unlocking and relocking of the fixation device. The axis for tensioning of the cable is coincident with the locking axis. A cerclage inserter instrument is disclosed.

The invention is directed in various aspects to a compression fixation system that includes coupling components and locking assemblies for connecting two or more separate elements in a fixed arrangement. For example, the system is useful for connecting and compressing two or more elements selected from bones and bone fragments. The system includes a substantially linear coupling component, such as, for example, a Kirschner Wire ("K- Wire") and locking assembly elements that engage with the coupling component in a coaxial orientation. The coupling component includes at least a linear portion and an anchor portion, the anchor portion configured to be fixed within or adjacent to a first one of the elements to be connected and the linear portion configured to be fixed adjacent to a second one of the elements to be connected, where the locking assembly is attached coaxially with and locked against the second element to achieve compression and fixation.

The present invention relates to a system, method, device and kit which utilize a modular pedicle screw implant that does not require the use of a guide wire for implantation and can be assembled in situ. The system, method, device, and kit include a surgical instrument for implanting a modular pedicle screw comprising a cannula coaxially aligned with a modified trocar style instrument or surgical shaft. The surgical shaft contains a first member of a surgical shaft-inner pedicle bone screw member joint, illustrated herein as a locking taper, for securing an inner member of a pedicle screw thereto. The inner member of a pedicle screw is further adapted to couple to an outer pedicle sheath. In combination, the inner member of a pedicle screw and the outer pedicle sheath form a solid modular pedicle screw.
