The circular openings each form a cavity (30) with walls (31) extending between the upper and lower plate member surfaces (16, 18), the cavities having smaller diameter portions (32) at the plate member surfaces (16, 18) and a larger diameter portion (34) therebetween. The cavities are configured in the shape of two inverted cones (31a, 31b) with matching base circles (31c) for allowing angulation of an implanted bone screw (14). A plurality of bone screws (14) are provided, each having a threaded first end portion (36) and an ellipsoid-shaped enlarged second end portion (40). A locking member (48) is also provided which connects to the second end portion (40) of each bone screw (14) for expanding the second end portion (40) so as to grip the cavity wall (31) of the plate member (12).

The present invention provides plate systems for distracting and fixating vertebral bodies of animal (preferably human) spines. In some embodiments, the system includes: a biocompatible plate with at least two holes, at least two biocompatible distraction screws for insertion through the holes, each screw having an expandable head, a bore and a threaded shank, and at least two biocompatible fixating screws with threaded shanks. Insertion of the fixating screws into the distraction screws causes the expandable heads to expand and lock to the biocompatible plate. Optionally, the plate and the expandable heads comprises mating teeth configured to lock the expandable heads to the plate upon expansion of the expandable heads. The present invention also relates to use of the plate system to distract and fixate vertebral bodies.

An orthopedic assembly is described that comprises an orthopedic device, an anchor, and a locking mechanism. The orthopedic device can be a plate member having an aperture that is configured to receive the anchor. The anchor can include a head, neck and shank portion. The head portion can include a plurality of arms separated by grooves that are capable of splaying. The assembly is configured such that when the locking mechanism is inserted into the head portion, this causes expansion of the arms of the head. This expansion locks and secures the anchor to the orthopedic device. Various instruments are provided that can deliver the locking mechanism to the anchor, and can provide impact to lock functionality.

A fixation plate system includes a plate with elongated slots, and first and second anchors for attaching the plate to vertebrae. The first and second anchors have different head configurations that are selected by the surgeon to control how settlement occurs at each fused segment. The head of the first fastener is insertable in any of the slots and resists translation. The head of the second fastener is insertable in any of the slots in free sliding engagement to allow translation of the head in that slot. In one method of use, the surgeon selects a segment where settling needs to be limited, and uses the first fastener to fix the plate near the segment and limit settling at that segment. The first and second fasteners have a built-in spring that assists in securing the fasteners in the plate.
