A method, code and system for planning the treatment a lesion on or adjacent to the retina of an eye of a patient are disclosed. There is first established at least two beam paths along which x-radiation is to be directed at the retinal lesion. Based on the known spectral and intensity characteristics of the beam, a total treatment time for irradiation along each beam paths is determined. From the coordinates of the optic nerve in the aligned eye position, there is determined the extent and duration of eye movement away from the aligned patient-eye position in a direction that moves the patient's optic nerve toward the irradiation beam that will be allowed during treatment, while still maintaining the radiation dose at the patient optic nerve below a predetermined dose level.

An eye contact device and assembly for stabilizing a human eye at a selected position in an external coordinate system is disclosed. The device includes an eye-contact member having an inner contact surface for contacting a front surface of an eye, and a vacuum port formed within the eye-contact member and communicating with the eye-contact member's contact surfaces, by which a negative pressure can be applied between the eye and the contact surface, to stabilize the position of the eye with respect to the device. The device, which is adapted to be biased against the patient's eye, to secure the device to the eye, may be pivotally coupled to a positioning arm, and may carry one or more beam-directing elements by which the position of the device in an external coordinate system can be determined.

A method of performing microbeam radiation therapy (MRT) includes delivering a dose only to selected tissue in a target volume (10) with continuous broad beam, first, by interleaving arrays of microplanar beams (30,36) only at the target (10). Administered contrast agents can supplement the effect by preferentially increasing the target dose relative to dose in normal tissue. A broad beam effect is alternatively created using non-interleaving microbeam array(s) with scattering agents administered to selected tissue that preferentially increase valley dose (69) within target to approximate broad beam. The methods of interleaving microbeams are also applied to treat diseases and conditions by ablating at least a portion of selected tissue, or by damaging blood-brain barrier for efficient drug and/or cell administration. A system for performing interlaced microbeam radiosurgery preferably includes two orthogonal radiation source arms (102) for producing and interleaving microbeam arrays (30,36) at the target volume (10). The methods treat tumors, pain, epilepsy, and neurological diseases.

Age-related macular degeneration is treated by radiation delivered from a miniature x-ray tube (20) inserted via a catheter(12) around the globe of the eye, to a position behind the macula (16). Methods are described for properly locating the catheter (12) and x-ray tube (20), using illumination on the catheter (12) and viewing through the front of the eye, or sensors on the catheter and a scanned beam shone from the front of the eye. Fluorescent excited by x-rays can also be used. Also described are methods and devices for immobilizing the probe once properly located in the eye, for standoff of the x-ray tube (12) from the target tissue, and for achieving prescription radiation dose in the choroid (22) while eliminating dose to adjacent tissues. The x-ray treatment can be enhanced using a radiosensitizing drug, and can be combined with PDT.
