An electrosurgical instrument provides pressurized gas to a surgical site and includes a hand-held applicator having a housing with proximal and distal ends and a tube defined therethrough. The tube is adapted to connect to a source of pressurized gas and is configured to selectively deliver a volume of pressurized gas to a surgical site. An actuator is also included which is configured to selectively regulate the volume of pressurized ionizable gas flowing through the tube. The actuator selectively regulates the flow of the gas through the tube. The actuator is positionable from a first position which allows a first predetermined volume of pressurized ionizable gas to flow through the tube for dissection purposes to at least one subsequent position which allows a different volume of pressurized ionizable gas to flow through the tube for coagulation purposes. The actuator, when disposed in the at least one subsequent position, operatively communicates with an active electrode to ionize the different volume of pressurized ionizable gas prior to the different volume of pressurized ionizable gas exiting the tube.

PROBLEM TO BE SOLVED: To provide a laser transmission line, a laser therapy instrument and a laser therapy system which can conduct laser light and assist gas without reducing transmission efficiency of the laser light.SOLUTION: The laser transmission line 70 is formed in the shape of a long item being hollow inside and includes a hollow waveguide 80 which conducts the laser light 57a for therapy and an exterior tube 71 which allows insertion of the hollow waveguide 80 therethrough. Here an assist gas conduction line 77 which allows conduction of the assist gas 59a having laser light absorptivity and living body absorptivity is formed between the hollow waveguide 80 and the outer peripheral surface 71a of the exterior tube 71.

Methods and systems for determining patient specific treatment parameters for delivering vapor to the lung to treat lung tissue. In some embodiments vapor is delivered to the lung to cause coagulative necrosis, inducing fibrosis and thereby reducing the volume of at least one segment of the lung. The delivery parameters can be adjusted depending on the desired degree of injury to be induced in the lung tissue.
