Abstract: An implantable medical device system receives a cardiac electrical signal produced by a patient's heart and comprising atrial P-waves and delivers a His bundle pacing pulse to the patient's heart via a His pacing electrode vector. The system determines a timing of a sensed atrial P-wave relative to the His bundle pacing pulse and determines a type of capture of the His bundle pacing pulse in response to the determined timing of the atrial P-wave.

Abstract: This disclosure is directed to devices and techniques for classifying of pacing captures to evaluate effectiveness of pacing by a pacing device, such as an implantable medical device (IMD). An example system includes stimulation circuitry to generate a pacing stimulus, sensing circuitry to sense an evoked response after the pacing stimulus, and processing circuitry. The processing circuitry determines classification features from the evoked response and applies the classification features to a classification model, the classification model generated by a machine learning algorithm using one or more test sets comprising a plurality of sample evoked responses for each of a plurality of classifications. Based on the output of the model, the processing circuitry classifies the evoke response as one of the plurality of classifications.

Abstract: An IMD system receives a near field His bundle electrical signal produced by a patient's heart via a first sensing electrode vector and a far field cardiac electrical signal via a second sensing electrode vector different than the first sensing electrode vector. The IMD system generates His bundle pacing pulses delivered to the patient's heart via a His pacing electrode vector and determines a type of cardiac capture evoked by a His bundle pacing pulse.

Abstract: An implantable medical device (IMD) performs, within a first predetermined time following an implantation, a first device test sequence over an evaluation period. The device test sequence includes at least two of: detecting an impedance for at least one electrical path having at least one electrode, and comparing the impedance to a first predetermined impedance threshold to determine a connection status of the IMD; comparing, over an electrogram (EGM) test period, at least one EGM event of the patient against a first predetermined EGM event threshold; determining a first pacing capture threshold of the IMD; and detecting at least one clinical or patient-specific physiologic metric, and comparing the physiologic metric to a first predetermined physiologic metric threshold. The IMD transmits within a second predetermined time a status signal to an external device indicating a status of at least one of the diagnostic tests in the first device test sequence.

Abstract: A medical device is configured to sense a cardiac electrical signal and determine from the cardiac electrical signal at least one of a maximum peak amplitude of a positive slope of the cardiac electrical signal and a maximum peak time interval from a pacing pulse to the maximum peak amplitude. The device is configured to determine a capture type of the pacing pulse based on at least one or both of the maximum peak amplitude and the maximum peak time interval.
