    摘要: In various surgical techniques, a bipolar forceps can be used to seal a vessel in two locations such that the vessel can be incised at a location positioned intermediate the two seal locations. The bipolar forceps can include a cutting element which can be configured to incise the vessel. In various embodiments, the cutting element can include a sharp edge which can be moved relative to the vessel. In at least one embodiment, the cutting element can be electrically connected to a source of energy. The bipolar forceps can include first and second electrodes positioned within first and second jaw members, respectively, wherein at least one of the jaw members can include a substantially tapered profile and can be configured to pull the vessel away from the surrounding soft tissue. Such jaw members can include ridges, teeth, and/or a textured outer surface configured to grip the soft tissue and/or vessel.

In one embodiment, an electro-surgical device is provided having a housing with a hollow interior adapted to receive tissue, such as an organ. The housing can be adapted to couple to a pressure source for suctioning tissue into the housing, and to an energy source for delivering energy to the tissue. The various elements of the device can be adapted such that the initially dispersed energy is passed through the tissue and is concentrated at a location, e.g., connective tissue, having a surface area that is less than a surface area of the electrode, thereby allowing the connective tissue to be dissected. Exemplary methods for dissecting tissue are also provided.

A guide wire structure is employed which comprises at least two guide wires each having a leading end portion which terminates in a leading end, the guide wires being connected to one another by a junction at or adjacent their leading ends, the guide wires have a first position in which the leading end portions are substantially parallel to one another, a second position in which the leading end portions are curved, and a third position in which at least one of the leading end portions forms a loop. The guide wire structure is steered through the gastrointestinal tract by selectively advancing or retracting a single guide wire or advancing more than one guide wire simultaneously, according to the path which is required to be followed.

    摘要: An endoscopic ablation system is provided for use with a flexible endoscope for the ablative treatment of diseased tissue on the interior lining a body lumen. The endoscopic ablation system includes a support member for supporting at least two electrodes that are electrically connected to a RF generator. The electrodes have a shape, size, and spacing that provide ablation between the electrodes, while minimizing ablation of tissue directly underneath the electrodes. The endoscopic ablation system can also include a sheath that fits over a flexible endoscope. A flexible coupling can join the support member to the sheath to facilitate intubation. The support member can include a side opening, and the sheath can include a seal, so that the aspiration means of the endoscope may be used to evacuate the air from inside the body lumen and pull the tissue to be treated into intimate contact with the electrodes.

    摘要: In the present invention, a cordless inductively coupled electrosurgical instrument is adapted to receive electrosurgical energy from a specially designed trocar or trocar adapter. In one embodiment of the present invention, an electrosurgical instrument includes a handle, an elongated tube and an electrosurgical end effector. The handle may include an actuator such as a trigger which is operatively connected to the end effector through the elongated tube. The elongated tube may be, for example, a closure tube which is adapted to close the end effector when the handle actuator is moved. The electrosurgical end effector is adapted to apply electrosurgical energy to biological tissue in order to effect treatment of the tissue. The elongated closure tube includes one or more inductor coils adapted to couple electrosurgical energy to the end effector. The one or more inductor coils are electrically connected to the end effector through one or more electrical conductors.

    摘要: A safety-shielded trocar having an obturator with a linear cutting edge surface defining a base width substantially less than the diameter of the safety shield is disclosed. When the trocar is inserted through tissue, the tissue dilates from the width of the linear incision to accommodate the size of the safety shield of the trocar, thus making it possible to provide an access opening greater than the incisional width made by the linear cutting edge surface of the obturator. In a preferred embodiment, the safety shield has a shield tip region which is asymmetric to facilitate the dilation of the tissue as the trocar is inserted.
