    摘要: Apparatus and method are described for introducing an imaging catheter to the coronary vasculature. A guiding catheter is introduced so that the distal end of the guiding catheter engages a coronary os. The distal end of the guiding catheter is shaped so that a mark on the distal end is oriented in a predetermined orientation relative to the coronary vasculature. An imaging catheter is then introduced through the guiding catheter and an image of the mark is produced with the imaging catheter while in the guiding catheter. In this manner, the relative orientation of the produced image and the coronary vasculature is known.

    摘要: A method for imaging an organ or internal structure within a body to obtain three-dimensional reconstruction of the organ or internal structure. The method includes steps of providing an intraluminal ultrasound imaging system capable of obtaining echographic images of the surrounding of a catheter imaging tip located inside the body of a patient, and a catheter tracking system comprising at least one ultrasound transducer mounted adjacent the imaging tip of an intraluminal ultrasound catheter for insertion into a patient, a plurality of transducers located away from said intraluminal ultrasound catheter so as to form a reference frame, and circuitry for calculating positions of the plurality of frame transducers relative to each other and positions of said catheter tip transducers relative to said plurality of frame transducers. The intraluminal ultrasound catheter imaging tip is positioned within a region of interest inside the body of the patient and a plurality of echographic images are obtained during deliberate movement or pull-back of the catheter, each image corresponding to an echographic data set. The position of the catheter axis with respect to each echographic data set obtained during pull-back is recorded. The position, and optionally the velocity of the catheter tip are recorded. The recorded position of the catheter tip can be used to calculate a catheter pull-back trajectory in three-dimensional space. The images are stacked around the catheter pull-back trajectory by positioning the origin of a first image at a first recorded position, and positioning subsequent images at their respective distances from the first image, wherein each echographic data set is positioned so that the catheter axis is aligned with the catheter pull-back trajectory.

    摘要: An intraluminal imaging catheter system for viewing segments of vessels downstream of the catheter itself is provided. The catheter system incorporates an ultrasonic transducer pivotally mounted adjacent the catheter's distal end. A flexible drive means disposed within a flexible tubular member adapted for insertion into the vessel causes the transducer to pivot back and forth to scan a segment of the vessel. Optionally, the forward viewing catheter system of the present invention may be combined with a work element such as a biopsy tool or other device for use in conjunction with the imaging capability.

    摘要: The present invention provides an improved drive shaft for use in a catheter system having a rotating imaging or interventional work element. A drive shaft according to the present invention is formed at least in part of a material having super elastic properties. This material provides an advantageous combination of column strength and torsional rigidity with the ability to sustain substantial bending without permanent deformation. Preferred embodiments are formed as hollow tubes to allow electrical conducting wires to be disposed within the body of the drive shaft. Some embodiments combine relatively rigid proximal segments with relatively flexible distal segments to allow for entry of the work element into narrow and twisting regions of a patient's vascular system.

A work element is secured to a distal end of the lumen while a coupling member for rotating the cable secured to the proximal end thereof. The cable includes a more flexible end located within the distal region of the catheter and a less flexible section located within the proximal region of the catheter. In this way, the drive cable is able to transmit torque sufficiently from the proximal end to the distal end even when the catheter is subject to tortuous twisting and bending as a result of positioning within confined regions of the body, such as the vascular system.

Aspiration means and flushing means are connected to the region of grinding to remove dislodged debris. A preferred embodiment has the abrasive surface of a rotatable head comprised of diamond powder, fused silica, tungsten carbide, aluminum oxide, or boron nitride. Another preferred embodiment has an ultrasonic transducer affixed to the head to permit transluminal imaging of the site of occlusion.
