    摘要: A system is provided for tracking and maintaining vascular access medical records across a continuum of healthcare facilities, facilitators or providers having respectively compartmentalized vascular access records. The system centrally aggregates vascular access records in association with a patient, with the vascular access records including at least past vascular access services rendered across the healthcare continuum. The system provides access by third party vascular access service providers to the vascular access records, responsive to new vascular access service requests for the patient from any one of the plurality of the healthcare entities. The system further enables supplementing the vascular access records to reflect results from the new vascular access service request, and/or post-service results, wherein the results are available for access in association with subsequent vascular access service requests for the patient by any one of the plurality of healthcare entities.
