    摘要: An implantable cardiac lead system suitable for placement in the coronary sinus region of the heart. The lead system comprises a lead having two or more non-helical bends in its distal portion. The two or more non-helical bends cooperate to prevent the lead from dislodgment or displacement inside the coronary sinus. The lead system may further comprise a stylet suitable for steering the lead into at least one of the coronary sinus vein, great cardiac vein, left marginal vein, left posterior ventricular vein, and small cardiac vein. The stylet is tapered in its distal portion to provide enhanced maneuverability and steerability inside the coronary sinus region. The lead system may also comprise an introducer which aids in introducing the lead into the heart.

    摘要: A single chronic implantable cardiac lead for use in the coronary sinus region of the heart provides both atrial and ventricular pacing and defibrillation therapy. The lead includes an elongated lead body having a distal end and a proximal end, a first electrode assembly including a ventricular pacing electrode and a ventricular defibrillation at the distal end, a second electrode assembly proximal to the first electrode assembly including at least one atrial pacing electrode and an atrial defibrillation electrode, and a further defibrillation electrode proximal to the second electrode assembly. The electrode assemblies and further defibrillation electrode are spaced apart so that when the lead is implanted within the coronary sinus region of the heart with the first electrode assembly adjacent the left ventricle, the second electrode assembly is adjacent the left atrium and the further defibrillation electrode is within the right atrium and/or the superior vena cava.

    摘要: An implantable cardiac stimulation device which determines stimulation based upon the patient's body position and activity level while eliminating special implantation or calibration procedures. To eliminate such special implantation and calibration procedures, the stimulation device correlates the patient's body position using a multi-axis DC accelerometer or other sensor during times of high activity and determines a patient's standing position value. During other times, the stimulation device compares the signals from the accelerometer to the standing position value to determine the patient's current body position. Based upon the current body position and the activity level, the stimulation device determines the necessary stimulation to deliver to the patient.

    摘要: An implantable lead for electrical stimulation of the body includes an elongated multi-lumen tube with a distal tip electrode having a longitudinally extending central bore. A cable conductor is received in one lumen of the multi-lumen tube for electrical connection to the tip electrode and an elongated polymeric tubular liner having a coefficient of friction in the range of 0.01 to 0.20 is received in another lumen of the multi-lumen tube generally aligned with the bore of the distal tip electrode for freely receiving a guidewire through the tubular liner and through the bore of the distal tip electrode. An electrically conductive proximal pin is attached to the multi-lumen tubing distant from the tip electrode and, in one embodiment, the cable conductor and the proximal end of the polymeric tubular liner are attached to the proximal pin. Initially, the guidewire is implanted into the body along a desired trajectory. With the polymeric tubular liner inserted, first the distal tip electrode, then the remainder of the multi-lumen tube, are slid onto the guidewire such that the guidewire slidably advances within the polymeric tubular liner. Thereupon, the multi-lumen tube is advanced along the guidewire until a desired site is achieved and the guidewire is removed from the body and the multi-lumen tube. In another embodiment, the distal tip electrode has no central bore but the elongated polymeric tubular liner is axially aligned with the distal tip electrode for freely receiving a stylet which is attached with the distal tip electrode.

    摘要: An implantable lead for electrical stimulation of the body includes an elongated multi-lumen tube with a distal tip electrode having a longitudinally extending central bore. A cable conductor is received in one lumen of the multi-lumen tube for electrical connection to the tip electrode and an elongated polymeric tubular liner having a coefficient of friction in the range of 0.01 to 0.20 is received in another lumen of the multi-lumen tube generally aligned with the bore of the distal tip electrode for freely receiving a guidewire through the tubular liner and through the bore of the distal tip electrode. An electrically conductive proximal pin is attached to the multi-lumen tubing distant from the tip electrode and, in one embodiment, the cable conductor and the proximal end of the polymeric tubular liner are attached to the proximal pin. Initially, the guidewire is implanted into the body along a desired trajectory. With the polymeric tubular liner inserted, first the distal tip electrode, then the remainder of the multi-lumen tube, are slid onto the guidewire such that the guidewire slidably advances within the polymeric tubular liner. Thereupon, the multilumen tube is advanced along the guidewire until a desired site is achieved and the guidewire is removed from the body and the multi-lumen tube. In another embodiment, the distal tip electrode has no central bore but the elongated polymeric tubular liner is axially aligned with the distal tip electrode for freely receiving a stylet which is attached with the distal tip electrode.

The tip electrode assembly is detachably connected to the lead body by means of a threaded coupling actuatable by a stylet inserted through the lumen of the lead body. The lead body may therefore be removed leaving behind a chronically implanted tip electrode assembly. The invention has particular utility in the context of passive endocardial heart pacing leads whose tip electrode assemblies include tines which become entangled in the trabecular network of the heart, and are therefore difficult to remove.

    摘要: Methods and systems for providing cardiac therapy are described. In some embodiments, methods and systems are configured to deliver atrial tachyarrhythmia therapy by confirming that a patient is asleep and thereafter administering the therapy. Sleep can be confirmed, in some embodiments, through the use of a histogram that can be calculated using one or more parameters that are monitored by an implantable stimulation device. Parameters can include both physiological and non-physiological parameters. In other embodiments, atrial tachyarrhythmia therapy is delivered by determining desirable times to administer such therapy. The connector defines a port for receiving the proximal end of the lead and a plurality of connector contacts. The number of connector contacts is greater than the number of terminals disposed on the proximal end of the lead. When the connector receives the proximal end of the lead, each of the terminals disposed on the proximal end of the lead makes electrical contact with at least one of the connector contacts of the connector and no two terminals make electrical contact with a same one of the connector contacts.

    摘要: A method of manufacturing a stimulation lead includes providing a lead body having an insulation section that defines a central lumen extending along the insulation section and conductor lumens extending along the insulation section and arranged around, and external to, the central lumen. The lead body also includes conductive contacts located along an axial end of the lead body and conductor wires with each conductor wire disposed within one of the conductor lumens and each of the conductor lumens having at least one of the conductor wires disposed therein. After providing the lead body, conductively at least one of the conductor wires to each of the conductive contacts; and placing non-conductive material into a portion of at least one of the conductor lumens of the lead body. A portion of the conductor lumens and at least a portion of the non-conductive material are disposed radially beneath the conductive contacts.
