Abstract:
A computer-based system for recording, storing, and accessing clinical documentation in an acute care setting is provided. In an embodiment of the invention, a single electronic database or repository for storing clinical patient notes, provides multiple points of read/write access via a user interface operating on one or more client computers that are in real-time communication with the repository.

Abstract:
A method of sharing patient information including creating a release authorization containing sufficient information to identify a patient and information authorized for transmission, which is a subset of information stored by an electronic health record (EHR) entity for that patient; the release authorization associated with a patient or a person acting as a proxy for the patient; receiving a request from a recipient entity for information; identifying the subset of information associated with the release authorization to be transmitted from the EHR entity to the recipient entity; and transmitting the subset of information from the EHR entity to the recipient entity.
Abstract:
A system provides an information sharing architecture that allows physically separate healthcare information systems, called "deployments," to share and exchange information. The collection of these participating deployments is referred to as the "Community," and systems within the Community sometimes store records for patients in common. The system allows participants in the Community to share information on data changes to these patients, and to reconcile concurrent and conflicting updates to the patient's record.
Abstract:
A system for providing users with decision support tools and features triggered by opening an order entry activity for a patient or by selection of orders within a graphical user interface. The system may be adapted to operate in conjuction with an integrated medical record system.
Abstract:
A patient health record system uses knowledge bases to dynamically build visit templates and suggest content based on the user's profile and current patient information. The visit template is available to the health care provider using the system, and is presented within an easy-to-use graphical interface comprising a navigation pane for moving one section of the template to another, and a visit information window that displays current visit information and allows the user to add and edit that information.
Abstract:
In accordance with one aspect of the invention, a patient-centered integrated health care record is provided including information related to health care delivery for a patient, and information related to health care delivery management for the patient. The integrated health care record may be used with a health care system to facilitate management of and to provide health care for patients. The health care delivery information and the health care delivery management information for patients may be stored within the UPR as patient records, with one patient record per patient. The information may be stored as formatted data, links to formatted data, or as selections from a list. In one embodiment, audit functionality is provided, where contact with data records is recorded as an audit trail. In another embodiment, security functionality is provided, controlling access to the integrated health care record. In another embodiment, a lock manager is provided, controlling write access to the integrated health care record. In accordance with another aspect of the invention, an electronic health care system includes a modular framework and a display in communication with the modular framework for providing a graphical user interface to a system user. The modular framework includes a plurality of activities, each activity providing an aspect of patient care, where the framework is adaptable for accepting additional activities forming a single integrated system. The graphical user interface is adaptable for displaying information corresponding to one or more of the activities, and includes a common menu format for communicating available operations in the graphical user interface, and common visual components for displaying information to a system user.
Abstract:
A system and method is provided for synchronizing a data set across a distributed, electronic, health record system which includes storing the data set at a first deployment, assigning an identifier to the data set, designating the first deployment as an owning deployment for the data set, and transmitting a portion of the data set, the identifier, and the home deployment designation to the second deployment. The method also includes causing the first deployment to transmit the portion of the data set, the identifier, and the owning deployment designation to the second deployment if it is determined that the data set should be transmitted to the second deployment, and causing the system to synchronize the data set between the first deployment and the second deployment.
Abstract:
An integrated system provides patients with secure, real-time access to their Personal Health Record and an Enterprise Health Information System (PHR and EHIS, respectively). Access may be provided by way of the Internet and via a Personal Health Portal (PHP) web page. From the secure PHP web page, patients can view information created and maintained by their health care providers and their affiliated staff. The patients can also request services and information from their health care providers and affiliated staff, directly access EHIS-related services, such as scheduling an appointment, scheduling, paying a bill, enrolling in a class, completing insurance and other forms, and viewing information and Internet services that are relevant to their particular health status.
